Definition and scoring system of molar incisor hypomineralization: A review

Abstract: Objectives: A definition and classification system for molar incisor hypomineralization (MIH) was first suggested by the European Academy of Pediatric Dentistry (EAPD) in 2003. Since then, there has been growing interest in these mineralization defects due to esthetic and cariogenic susceptibility concerns. The aim of this review was to evaluate studies on MIH in an attempt to provide evidence for consistency and standardization of MIH definition and severity scoring reporting by clinicians and researchers.Met… Show more

“…Some authors have suggested MIH to be an autosomal recessive condition or localized amelogenesis imperfecta with possible association with genes related to enamel formation such as AMBN, TUFT1, and TFIP11. [ 19 ] MIH may be acquired as a result of prenatal, perinatal, and postnatal illness; low birth weight; antibiotic consumption; and toxins from breastfeeding. [ 1 11 ] Certain drugs such as chemotherapeutic, antibiotics, asthma, and antiepileptic drugs can cause MIH.…”

“…Clinical management of MIH is difficult due to the rapid development of dental caries, limited cooperation of children, and repeated breakdown of restorations [ 11 ]. MIH can affect both aesthetics and cariogenic susceptibility and is considered a global dental problem [ 12 ].…”

“…These enamel defects tend to fracture during eruption and cause not only dentin hypersensitivity but also increased susceptibility to caries. [5] Therefore, given the burden of disease and high prevalence of MIH in some populations, [6][7] the cost of these conditions to the affected families as well as the general community can be substantial.…”

“…[8] In addition, while MIH diagnosis is reported by some studies based on the presence of at least one affected first permanent molar (FPM), other studies have based the diagnosis on affected anterior teeth when an affected FPM was not present. [6] The presence of hypomineralization in other teeth including the second primary molars, cusp tips of second permanent molar and canines have been reported. [5] Furthermore, evidence from several studies and systematic reviews indicate hypomineralized second primary molars are highly predictive of MIH with a 33% increase in incidence of MIH if an affected primary tooth is diagnosed.…”
