Demyelinating neuropathy with anti‐CRMP5 antibodies predating diagnosis of breast carcinoma: Favorable outcome after cancer therapy

Abstract: This case widens the range of paraneoplastic manifestations of breast carcinoma to include anti-CRMP5 antibody-positive sensory ataxic demyelinating neuropathy, which, in this patient, improved after cancer treatment.

“…We identified 6 published cases of CIDP associated with breast cancer. 24,37,[49][50][51] In all of 5 cases with documented history, the neuropathy preceded the cancer diagnosis by months to years. Sensory ataxia predominated in 2 cases, and cranial nerve involvement was reported in 1.…”

“…Other antibodies are also associated with paraneoplastic neuropathy. In one example, a patient developed a sensory-predominant, mildly ataxic demyelinating neuropathy 2 years preceding eventual diagnosis of breast cancer [27]. She also showed elevated cerebrospinal fluid protein and a serum IgG kappa paraprotein.…”

“…Other antibodies, however, have been and continue to be identified and associated with PN, including anti-Yo, anti-Ri, and antiamphiphysin. 34,[37][38][39] In addition to small cell lung cancer, other associated malignancies include adenocarcinomas, lymphomas, thymomas, breast cancer, and gastrointestinal cancers. As new antibody and oncologic associations continue to be discovered, it is paramount to screen broadly for various paraneoplastic antibodies and to retain a high index of suspicion for paraneoplastic disease and occult malignancy in patients with unexplained PN.…”
