Dental caries prevalence in individual tooth in primary and permanent dentition among 6-12-year-old school children in Shimla, Himachal Pradesh

Objectives To assess the association between dental sealant placement and subsequent restorative treatment of permanent first molars over time. Methods We analysed Wisconsin Medicaid claims data from 2001 to 2009 for children aged 6‐16 years. Children entered the study cohort at age 6 and were censored if Medicaid eligibility was lost for >31 days. A fixed effects analysis via a Cox proportional hazards model, stratified by individual, was used to estimate the time‐averaged and time‐dependent effects of sealant placement on dental treatment defined as any restorative, endodontic or surgical procedure. Results A total of 185,262 children with permanent first molars who turned 6 years enrolled in Medicaid were examined. Sealant placement was higher for teeth #16 and 26 (5.42 and 5.46 per 100 person‐years (100PY), versus 5.29 and 5.31/100PY for #36 and 46, respectively. The average rate for restorative treatments had the opposite pattern, with lower rate for teeth #16 and 26 (1.78 and 1.72/100PY) versus teeth #36 and 46 (2.14 and 2.12/100PY), respectively. In the fixed effects regression model, the hazard of dental treatment was substantially lower after sealant placement on a tooth, with time‐averaged hazard ratio HR = 0.23 (95% CI 0.21‐0.25, P < .001) versus before sealant. The largest effect was in the first year after sealant placement (HR = 0.13, 95% CI: 0.11‐0.14), which decreased over time (HR = 0.50, 0.59 and 0.74 in years 2, 3 and 4, respectively), and was not statistically significant in later years. Conclusions This study demonstrates that permanent first molar sealant placement delayed subsequent dental treatments in children enrolled in Medicaid.

BACKGROUND Caries Assessment Spectrum and Treatment (CAST) instrument index was introduced for full assessment of dental caries, as it covers from no lesion to advanced stage progression of caries. CAST in comparison to WHO criteria allows a more detailed reporting of stages of dental caries. Aims and Objectives: To determine the levels of similarity and difference between the CAST instrument and WHO criteria on the basis of caries prevalence, dmft/DMFT counts and reporting of result in 6-12 years old school children in Shimla city of Himachal Pradesh. MATERIALS AND METHODS A cross-sectional study was carried out among 623 school children aged 6-12 years in Shimla city. DMFT (Decayed, Missing, Filled Teeth) counts were obtained according to the WHO criteria and the CAST instrument. Descriptive statistics, test and correlation was done using SPSS version 19. The sample size was taken for convenience during the study. RESULTS 623 children were examined. The prevalence of dental caries calculated according to the CAST instrument and the WHO criteria for the primary dentition was 74.47% and 76.47% and for the permanent dentition it was 9.93% and 10.11%. CONCLUSION The CAST instrument provided similar DMF counts as the WHO criteria in all age groups. Presentation of results from use of the CAST instrument in comparison to WHO criteria allowed a more detailed reporting of stages of dental caries, which will be useful for oral health planners.

Primary second molar relations in preschool-aged children

Introduction:The primary second molar relation is also called the terminal plane, which is the relation between the distal surface of the maxillary and mandibular second primary molars in a centric occlusion position. One of the most important characteristics of the ideal occlusion development on the primary dentition is if the primary second molar relation classified as the flush terminal plane. This relation plays a significant role in determining the relation in permanent molar because the eruption of the first permanent molars are guided by the distal surface of the maxillary and mandibular second primary molars. The purpose of this study was to found out the clinical description of the primary second molar relation among the preschool-aged children in four kindergartens in Banda Aceh. Methods: This study was a descriptive study with cross-sectional study approach. The subject selection method was performed with a purposive sampling method involving as much as 37 subjects, which were students of State Kindergarten of Pembina 1, State Kindergarten 2, State Kindergarten 3, and State Kindergarten of Adidarma 4, Banda Aceh. The examination was performed by using the intraoral mirror when the teeth were in the centric occlusion position. Decisions regarding the primary second molar relation were made by the bilateral occlusion (left and right). Results: The flush terminal plane was found in 45.9% of all respondents, the mesial step was 54.1%, and the distal step was 0.0%. Conclusion: The primary second molar relation among preschool-aged children generally had the mesial step.
