Aims. The primary aim was to investigate variables affecting compliance in pregnant women recommended for genetic consultation for abnormal screening test results, family predisposition or medical history. Our main focus was on a women's knowledge of particular screening tests, their initial feelings and changes in these feeling with time, as well as variables relevant to further decision making. Methods. We used an anonymous questionnaire based on previous qualitative research. The questions were formulated by a medical geneticist, and the questionnaires were distributed prior to prenatal screening tests performed by doctors or trained nurses. The research cohort consisted of 271 women aged 16-42 years. Six hypotheses were tested using the statistical programme STATISTICA; significance levels were set to P<0.05. Results. The questionnaire results showed insufficient knowledge. The women were confused about invasive, screening and ultrasound tests. Genetic test recommendation was largely associated with stress in these patients. Between recommendation and consultation, the women mostly looked for support from their partners. There was a surprisingly low percentage of women who looked for help from their medical specialists and a surprisingly high percentage of those who did not seek any help at all. Conclusion. Women's distress can be reduced if the information about recommended genetic consultation is conveyed correctly and this can also help them make the right informed decision about their future course of action.

Objective: Although prenatal ultrasound (US) is a common clinical undertaking today, little information is available about women's experience of the procedure from the perspective of women themselves. The objective of this study was to explore women's experience of undergoing a routine prenatal US examination associated with an unexpected fetal diagnosis.Study Design: Qualitative methods were used to explore the prenatal US experience of 13 women. Five women were given unexpected news of multiple pregnancy and eight women were given unexpected news of congenital fetal abnormality. One in-depth audio-taped interview was conducted with each woman. Content analysis of interview data identified themes common to women's experience of US.Results: Identified themes of women's experience of routine prenatal US examination associated with an unexpected fetal diagnosis are: experiencing the setting, sensing information, feeling connected/ disconnected, the power of the image, and communication rules. Conclusions:Women's experience of prenatal US examination is influenced by physical and environmental factors and by the behaviors of the US examiner. Behaviors of the examiner contribute to a woman's labeling of the US experience as positive or negative. Women identify being objectified by the examination and experience poor communication patterns after a fetal US diagnosis. Women's description of the US screen image as a baby suggests it is a powerful influence on subsequent clinical and ethical decision-making about the pregnancy. Study of US as a perinatal diagnostic tool has focused on whether US improves perinatal outcomes, 3-6 the psychological effect on women and men of such an examination, 7-9 perception and receipt of information, 10-14 and the experiences of staff who perform US examinations.15,16 A 1998 Cochrane review of routine US 17 focused only on physical outcomes, as did a later work. 18Reviewers of the work suggested a lack of research on women's experience of this type of procedure. 19Other researchers have focused on specific aspects of breaking bad news in perinatal situations and the sequelae of such an action. Alkazaleh et al.20 surveyed 117 women with pregnancy complications detected sonographically. Sixty-seven women responded to their survey of broad characteristics of bad news transmission, reporting what women found helpful after being given bad news. RESUMOObjetivos: descrever os processos emocionais vivenciados com a interrupção da gestação após o diagnóstico de malformação fetal letal. Métodos: foram entrevistadas 35 gestantes cujo feto era portador de malformação letal e que realizaram a interrupção da gestação após solicitação de autorização judicial. A malformação fetal mais freqüente foi a anencefalia (71,5%). As pacientes foram submetidas à entrevista psicológica aberta logo após o diagnóstico da malformação fetal para que pudessem expressar os sentimentos desencadeados e para promover reflexão sobre a solicitação da interrupção da gravidez. O tempo médio de espera pelo deferimento do pedido judicial foi de 16,6 dias. As que solicitaram e tiveram o aborto realizado foram convidadas a retornar para a segunda entrevista psicológica 30 a 60 dias após o procedimento, quando foi aplicado questionário semidirigido para identificar os aspectos emocionais vivenciados e descrever os sentimentos despertados. Resultados: trinta e cinco pacientes foram entrevistadas após o aborto. Quanto aos sentimentos vivenciados na decisão pela interrupção, 60% relataram como negativos, 51,4% afirmaram que não tiveram dúvidas quanto à decisão tomada e 65,7% informaram que a própria opinião foi a que mais pesou na decisão. A maioria das mulheres (89%) afirmou apresentar lembranças do que viveram com certa freqüência, 91% afirmou que adotariam a mesma atitude em outra situação semelhante e 60% diriam para interromper a gestação caso alguém perguntasse seu conselho, numa mesma situação. Conclusões: as angústias vivenciadas demonstram que o processo de reflexão é de fundamental importância para decisão consciente e posterior satisfação com a atitude tomada. O acompanhamento psicológico permite revisão dos valores morais e culturais para auxiliar a tomada de decisões visando minimizar o sofrimento vivido. Methods: thirty-five pregnant women who underwent termination of pregnancy for lethal fetal anomaly after judicial permission were interviewed. The most frequent fetal malformation was anencephaly (71.5%). The patients were submitted to an open interview as soon as the diagnosis of fetal malformation was confirmed, allowing them to express their feelings and stimulating them to think about asking for termination of pregnancy. The mean time spent until the judicial agreement was 16.6 days. The women who requested and were submitted to the procedure of abortion were invited to return for psychological evaluation after 30-60 days. At this moment, a semi-structured interview was performed to find the emotional aspects and feelings that existed. Results: thirty-five patients were interviewed.
