V aginitis is a common medical problem in women that is associated with substantial discomfort and frequent medical visits. Roughly 5 to 10 million office visits per year are attributed to vaginitis. 1 Research in recent years has increased our understanding of the disease process and its potential sequelae and resulted in improved diagnostic and treatment modalities. 2-6 Nevertheless, adequate diagnosis and treatment of vaginitis in the office setting often is problematic for practitioners. 2,5,7-9 Moreover, the availability of over-the-counter medications for fungal vaginitis has increased the likelihood that women will come to medical attention with partially or inadequately treated infections. 10 We will focus on the common infectious causes of vaginal discharge-yeast infection, bacterial vaginosis, and trichomonas infection, emphasizing new developments in diagnosis, associated complications, and treatment of these infections.The initial evaluation of vaginal discharge requires an understanding of physiologic vaginal discharge and what differentiates it from abnormal, pathologic discharge. Substances from the vulvar, sebaceous, sweat, Bartholin's and Skene's glands, as well as exfoliated cells, cervical mucus, and secretions of the endometrial cavity and fallopian tubes constitute the normal physiologic secretions of the vagina. These secretions pool in the posterior fornix and do not adhere to the vaginal walls. 11 The pH of normal vaginal secretions in women of childbearing age is between 3.8 and 4.5. The presence of sperm, blood, amniotic fluid, or cervical mucus raises the vaginal pH. 11 The amount and fluidity of the discharge can vary over the menstrual cycle. Cervical mucus becomes more fluid around ovulation, and women frequently mistake this change in consistency for an abnormal discharge. Evaluation of affirm VP Microbial Identification Test for Gardnerella vaginalis and Trichomonas vaginalis

A commercial system (Affirm VP Microbial Identification Test; MicroProbe Corp.) for detection of vaginal pathogens was evaluated with 176 consecutive women attending a sexually transmitted disease clinic for genital complaints. Vaginal swab specimens were used for culture of GardnereUa vaginalis and Trichomonas vaginalis, preparation of a vaginal smear for Gram stain interpretation, and wet mount evaluation. An additional swab was used to evaluate the 30-min nonisotopic oligonucleotide probe test. The automated probe system detected G. vaginalis in 69 (95%) of 73 women having >5 x 105 CFU of G. vaginalis per ml by culture, and 20 (43%) of 47 specimens with c5 x 10' CFU of G. vaginalis per ml. There were three false positives and four false negatives for the Affirm VP test compared with >5 x 10i CFU of G. vaginalis per ml. The probe system detected G. vaginalis in 57 (90%k) of 63 vaginal specimens from women having clue cells on wet mount examination, and in only 3 (3%) of 113 women without clue cells, suggesting that the Affirm probe for G.

Objective: Trichomonas vaginalis vaginal infections are often both asymptomatic and difficult to detect by current methods. We evaluated the ability of a newly developed polymerase chain reaction (PCR) assay to identify T. vaginalis in vaginal samples from pregnant and non-pregnant women. Methods: In the 1st study, we compared the prevalence of T. vaginalis detection by PCR and culture using Diamond's medium in 52 women with symptoms of vaginal infection. In the 2nd study, T. vaginalis was detected using PCR and wet mount microscopy in 131 asymptomatic pregnant women. Results: Among the women with symptoms of vaginitis, 7 (13.5%) were PCR-positive for T. vaginalis. Six of the PCR-positive women, but none of the PCR-negative women, were culture-positive for this organism. All but 1 of the women with candidal vaginitis or bacterial vaginosis were PCR-negative for T. vaginalis. Among the asymptomatic pregnant women, all of whom were negative for T. vaginalis by wet mount, l0 (7.6%) were PCR-positive for T. vaginalis. Conclusions: PCR offers a rapid and sensitive alternative to culture and microscopy for the detection of T. vaginalis vaginal infections in both symptomatic and asymptomatic women.
