BACKGROUND Celiac disease (CD) has been associated with gastrointestinal malignancies. However, the magnitude of the risk of pancreatic cancer (PC) associated with CD is much less clear, and risks have not been estimated from large populations. AIM To assess the risk of PC in CD patients. METHODS We conducted a population-based, multicenter, propensity score-matched cohort study with consecutive patients diagnosed with CD using the TriNeTx research network platform. We examined the incidence of PC in patients with CD compared with a matched cohort of patients without CD (non-CD, controls). Each patient in the main group (CD) was matched to a patient in the control group using 1:1 propensity score matching to reduce confounding effects. The incidence of PC was estimated using a Cox proportional hazards model with a hazard ratio (HR) and 95% confidence interval (CI). RESULTS A total of 389980 patients were included in this study. Among them, 155877 patients had a diagnosis of CD, and the remaining 234103 individuals without CD were considered a control cohort. The mean duration of follow-up for patients in the CD and control cohorts was 5.8 ± 1.8 and 5.9 ± 1.1 years, respectively. During the follow-up, 309 patients with CD developed PC, whereas 240 patients developed PC in the control group (HR = 1.29; 95%CI: 1.09-1.53). In the secondary analyses in the first year after diagnosis of CD, patients with CD were at a significant increase in risk for PC; 151 patients with CD had an incidence of PC compared with 96 incidences of PC among the patients in the non-CD control group (HR = 1.56; 95%CI: 1.20-2.01) and sensitivity analysis showed similar magnitude to the one generated in the primary and secondary analysis. CONCLUSION Patients with CD are at increased risk of PC. Risk elevation persists beyond the first year after diagnosis to reference individuals without CD from the general population.

Ultrasound-assisted dispersive liquid-liquid micro extraction (UA-DLLME) was proposed for simultaneous determination of trace levels of Vitamin B 1 (thiamine) and Vitamin B 9 (folic acid) in the human urine and serum samples. High performance liquid chromatography (HPLC) coupled with chemometrics method was employed to analysis the extraxtants. Objective: The optimal conditions of the extraction recovery, including pH of sample solution, type and volume of extraction solvent, disperser solvent type and volume, temperature and time of ultrasound, centrifugation time and ionic strength were were studied step-by-step via one-factor-at-a time procedure. Then significant variables were chosen and considered to further optimize using the hybrid Box-Behnken design -genetic algorithm. UA-DLLME method with HPLC-UV and utilizing hybrid Box-Behnken designgenetic algorithm to optimize the extraction process was presented for the first time in this study. Under the optimized extractions, relative standard deviations (RSD) of the analyses less than 4% (n= 3) and detection limit of 3.09-11.15 ng mL. Recoveries for both thiamine and folic acid were in the ranges of 88-107%. As the result, the UA-DLLME -HPLC coupled hybrid Box-Behnken design -genetic algorithm was successfully applied for the simultaneous determination trace levels of thiamine and folic acid in human urine and serum samples.

A new voltammetric method, based on the use of single‐use pencil graphite electrode, was developed for the quantitative determination of cefdinir by square wave voltammetry. Chemometric optimization strategy was used to select the suitable values for three experimental parameters (pH of the supporting electrolyte, frequency and square wave amplitude) by applying a 33 full factorial design model. The optimal voltammetric conditions were found to be pH 5.0 for supporting electrolyte (Britton‐Robinson buffer), 43 mV for square wave amplitude and 108 Hz for frequency. Square wave voltammograms of calibration, validation and unknown samples were recorded between 0.0–1400 mV under the optimized voltammetric conditions. Linear regression equation was computed in the linear working range of 0.5–20 μg/mL by using the relationship between the concentration and peak current at 600 mV. The optimized voltammetric method was validated by assessing the analysis results of validation samples. Then the proposed method was applied for the quantitative determination of cefdinir in three different pharmaceutical preparations, namely film‐coated tablets, effervescent tablets and powder for oral suspension. It was concluded that the proposed voltammetric method was suitable for the quantitative analysis of commercial pharmaceutical preparations containing cefdinir.
