Development of Quality Metrics in Ambulatory Pediatric Cardiology

Abstract: The American College of Cardiology Adult Congenital and Pediatric Cardiology (ACPC) Section had attempted to create quality metrics (QM) for ambulatory pediatric practice, but limited evidence made the process difficult. The ACPC sought to develop QMs for ambulatory pediatric cardiology practice. Five areas of interest were identified, and QMs were developed in a 2-step review process. In the first step, an expert panel, using the modified RAND-UCLA methodology, rated each QM for feasibility and validity. The … Show more

“…With regard to ambulatory services, we referred to the list of 18 measures established by Chowdhury and colleagues and included in the larger list of ambulatory measures currently endorsed by the ACC's ACPC Quality Network . A fundamental principle of the metric development work conducted by the ACPC Ambulatory Pediatric Cardiology Group was that metrics should target areas of practice in which the metrics could be supported by evidence‐based guidelines or widespread consensus.…”

“…Important quality measures have been developed through professional societies or national quality improvement collaboratives. As an example, quality in ambulatory pediatric cardiac care has been the focus of the American College of Cardiology's Adult Congenital and Pediatric Cardiology (ACPC) Quality Network, whose endorsed metrics form the basis of a web‐based data collection and reporting platform . Examples of established cardiac procedure‐based registries include the Society for Thoracic Surgery (STS) National Database and the IMproving Pediatric and Adult Congenital Treatments (IMPACT) catheterization Registry .…”

“…The council chose the following five areas on which to focus their efforts; chest pain, Kawasaki disease, tetralogy of Fallot, transposition of the great arteries after arterial switch, and infection prevention. The initial results of the overall effort were published earlier this year by Chowdhury et al The Infection Prevention Committee focused on areas in outpatient pediatric cardiac care where infection prevention may play a significant role. Surgical outcomes and related infectious concerns, while initially considered by the committee, were not addressed in order to focus on outpatient pediatric cardiology practice.…”
