Diagnosis and Management of Pancreatic Carcinoma During Pregnancy

Abstract: Delayed diagnosis and treatment are associated with high morbidity of both neonate and mother in cases of pancreatic cancer during pregnancy. Computed tomography scanning and carbohydrate antigen 19-9 levels are useful for diagnosis, after which radical surgery should be performed immediately in late pregnancy.

“…The mode of delivery has been a Cesarean Section in 16 of the 26 ongoing pregnancies (61.5%) [10,12,14,15,16,18,20,21,22,23,24,25,27,33,34,35]; an obstetric indication occurred for 8/16 (50%) cases, the remaining were performed to manage the pancreatic disease or due to deteriorating condition of the mother. A preterm delivery (delivery at a gestational age earlier than 37 weeks of gestation) occurred for 18/26 (69.2%) cases; 11/18 were induced in order to manage the pancreatic disease or due to deteriorating condition of the mother.…”

“…Diagnosis in pregnancy, however, is complicated by normal physiologic changes and fetal considerations. Transabdominal ultrasound is often insufficient to evaluate the pancreas and can be obscured by bowel displaced by the gravid uterus [14]. Contrast-enhanced CT scan is the method of choice for diagnosis and staging of pancreatic tumors in non-pregnant patients [5]; however, CT is often delayed in pregnancy, considering the risks of fetal radiation exposure.…”
