Diagnosis and Treatment of 26 Cases of Abdominal Cocoon

Abstract: AC can be categorized into two types; surgery is recommended for type II and part of type I with severe complications, but sometimes conservative therapy might be appropriate for type I. Laparoscopic surgery plays an important role in the diagnosis and treatment of AC. Furthermore, favorite health education, healthy diet and lifestyle are of significance in patients' recovery.

“…Surgery is often long and difficult, and recurrence of intestinal obstruction due to intestinal adhesion may occur after surgery. In asymptomatic or minimally symptomatic cases, if the diagnosis can be made with CT, it can be managed with conservative treatment [9]. In our case, the patient was first given conservative treatment.…”

“…In our case, it was determined that there was chronic pain in addition to acute abdominal pain. In addition to abdominal pain, symptoms such as nausea, vomiting, inability to pass gasstool, as well as increased abdominal pain and a mass found by hand in physical examination, which may be signs of peritonitis, may be encountered [9]. Clinical signs and symptoms may vary depending on the severity and duration of the disease, the underlying causes and the patient's immunological status.…”

“…[ 24 ] Additional appendectomy is worth recommending because the surgery would be difficult if appendicitis were to occur after the surgery. [ 25 ] Prophylactic application of an antiadhesive substance such as sodium hyaluronate between the intestinal loops before closing the abdomen may be useful in preventing the development of EPSBO.…”

“…While IESP has no known etiology, secondary sclerosing encapsulating peritonitis has been associated with peritoneal dialysis, recurrent peritonitis, luteinized thecomas, abdominal tuberculosis, autoimmune diseases and several other causes [3–5]. Preoperative diagnosis is difficult, owing to disease rarity, nonspecific imaging findings and its varied clinical presentation, which may range from abdominal pain to small bowel obstruction [3, 6]. This case report is a rare example of abdominal cocoon presenting as massive ascites in the absence of bowel symptoms or exposures that would contribute to secondary sclerosing peritonitis [7].…”
