Diagnosis of Cytomegalovirus (CMV) Polyradiculopathy and Documentation of In Vivo Anti‐CMV Activity in Cerebrospinal Fluid by Using Branched DNA Signal Amplification and Antigen Assays

Abstract: Branched chain DNA assay (bDNA), cytomegalovirus (CMV) antigen assay, and cerebrospinal fluid (CSF) viral culture were studied for their utility in the diagnosis of CMV polyradiculopathy and for documenting in vivo antiviral effects. CMV was demonstrated in 15 of 16 patients by bDNA assay, 15 of 16 by CMV antigen assay, and 11 of 15 by CSF culture. When clinical criteria and results of the other two assays were used as reference standards, the sensitivity of bDNA was 94% and 100% and the specificity 95.2% and … Show more

“…Also, pp65 antigenemia assay level of >100nuclei/200,000 cells has been associated with the occurrence CMV disease in 63% of patients, compared to 3% in antigenemia negative patients [160]. Additionally, the pp65 antigenemia from the CSF can be successfully used in confirmation of CMV polyradiculomyelopathy seen almost exclusively in AIDS patients [167, 168]. …”

“…We performed a very limited assessment of mono versus combined therapy in patients with CMV CNS disease, undergoing serial CSF examinations (Flood et al, 1997). Utilizing the bDNA assay, we compared the rate of decline of CMV signal in patients receiving GCV 10 mg/kg/d versus GCV 10 mgm/kg/d plus FOS 180 mg/kg/d.…”

“…The branched DNA assay is based on signal amplification rather than target-amplification (Urdea, 1994). This assay has been used to measure CMV DNA and HIV-1 RNA levels in the CSF of HIV-infected patients (Flood et al, 1997;Stingele et al, 2001).…”
