Diagnosis of Food Allergy

Abstract: Synopsis The prevalence of food allergies has been on the rise over the last two decades. Diagnosing food allergies can be complicated as there are multiple types of food allergies that have distinct clinical and immunologic features. Food allergies are broadly classified into IgE-mediated, non-IgE-mediated or mixed food allergic reactions. This review focuses on the clinical manifestations of the different categories of food allergies and the different tests available to aid the clinician towards an accurate … Show more

“…Whereas IgE-mediated allergies to cow's milk, egg, wheat and soy tend to be outgrown, atopic responses to peanuts, tree nuts and shellfish usually persist into adulthood 17 . The most common IgE-mediated food allergies vary somewhat by region and with dietary habits; for example, children in Ghana between 5 and 16 years of age are commonly allergic to pineapple, pawpaw, orange and mango, as well as to peanut 18 , whereas children in North America are commonly allergic to peanut, milk, egg, shellfish and soy 4 .…”

“…When IgE-mediated food allergy is suspected, measurements of total and allergen-specific serum IgE (sIgE) levels and skin prick tests (SPTs) are recommended to identify the causative food, although they are not in themselves diagnostic 16 . Increased sIgE levels may suggest food allergy, and sIgE thresholds with a 95% positive predictive value have been determined for some food allergens; for example, in peanut-allergic patients, the threshold sIgE level for peanut antigen is 14 kU A /L (U A = allergen-specific units; 1 unit ≈ 2.4 ng IgE) 17,92,93 . SPTs provoke allergen-mediated mast cell degranulation in the skin, leading to wheal-and-flare reactions.…”

“…IgE aracılı besin allerjisi tanısında en önemli araç besin alımı ile ortaya çıkan klinik hikaye varlığında deri testi veya kan testi ile besin spesifik IgE'nin gösterilmesidir (19). Her ne kadar altın standart besin provokasyon testi olsa da ilk basamakta spesifik IgE varlığının araştırılması hekime yol gösterir (20,21).…”

“…Besin allerjisi tanısında besin provokasyon testi altın standarttır (19), fakat özellikle 6 ay altında her zaman mümkün olmamaktadır. Bu çalışmada deri testinde duyarlanma saptanan her hastada ilgili besin diyetten çıkarıldığında atopik dermatit semptomlarında gerileme tespit edilmiştir.…”
