Differential Characteristics of AIDS Patients with a History of Imprisonment

Abstract: AIDS patients in Barcelona with prison histories tended to be younger, more likely to be IVDU, and to present with extrapulmonary tuberculosis as an AIDS-defining illness than other patient groups. Better survival appears to be related to age and AIDS-defining illnesses in the prison group. The fact that half the IVDU AIDS cases had prison histories has important implications for the care and prevention of HIV, tuberculosis, and drug abuse in comparable prison settings.

“…3 Intravenous drug users are the group at greatest risk for contracting AIDS 4 and tuberculosis is the first AIDS defining disease. 5 Similar patterns have been seen in several other cities and industrialised countries. 6 7 Studies measuring the prevalence of drug abuse done in other communities have proposed various methods, [8][9][10] which can be classified as direct or indirect.…”

“…The proportion of TB-related AIDS diagnoses among PWID ranged from 5 to 48% (Table 5; Amill et al, 2004;Castilla et al, 1995Castilla et al, , 1997Cayla et al, 1995;Chariyalertsak et al, 2001;Godoy et al, 1998;Jones et al, 1996;Laguardia and Merchan-Hamann, 2003;Schwoebel et al, 1995;Triol et al, 1996;Vall Mayans et al, 1997), highest in Spain and Portugal, the most TB-affected countries in Western Europe, and lowest in the UK, Puerto Rico and mainland US, where population TB rates are considerably lower (World Health Organization, 2010). Common predictors of TB as an AIDSdefining illness were male gender and acquisition of HIV through IDU (versus other transmission routes).…”

“…Information regarding risk factors for coinfections (HIV, HBV, HCV) is scarce, although the epidemiology of these three infections is well-known. The high prevalence of these infections (HIV, HBV, HCV) in Spanish prisons [17][18][19][20] offers a good opportunity to study the risk factors for coinfections as opposed to monoinfections. This was the major objective of this report.…”
