Differential diagnostic performance of acoustic radiation force impulse imaging in small (≤20 mm) breast cancers: Is it valuable?

Abstract: To evaluate acoustic radiation force impulse (ARFI) inthe differential diagnosis of small (≤20 mm) solid breast lesions and identify the most efficient ARFI parameters. Conventional ultrasonography and ARFIwere performed in 120 patients with 121 small solid breast lesions. The area ratios (ARs) of the lesion on virtual touch tissue compared to B-mode were calculated. The shear wave velocity of the inner (SWVi) and boundary (SWVb) of the lesions and surrounding fatty tissue (SWVf) was measured. The ratio of SWV… Show more

“…The falsenegative rate of r-US in combination with mammography (9.7%) was higher than what we found for r-US alone (5.6%). Although the false-negative rates for m-US vary in the literature (3.6-7.5%) [20,25,26], we found the same value for r-US and m-US (5.6%). Two lesions were wrongly classified as BI-RADS 3 lesions by m-US and r-US, while histology revealed a DCIS and an invasive ductal triple negative breast cancer.…”

“…The diagnostic accuracy of r-US was 89.3% which corresponds to the highest values found for m-US [19,20]. Similarly, PPV (69.6%) and NPV (98.3%) for r-US were in the range compared to published m-US data [20,21,[23][24][25].…”

“…The first excitation method involves applied manual compression, where the operator can exert pressure on the tissue with the ultrasound transducer to image superficial organs. The other excitation method requires the transducer to be held stationary while the internal physiological mechanisms (e.g., cardiovascular movement, respiration) induce the tissue deformation that is measured ( 42 ). This method is better suited to assess deeper internal organs as it is not dependent on the applied pressure of the operator.…”

“…Despite the advances in diagnostics and treatments, 20%–30% of patients with primary breast cancer experience recurrence and distant metastasis ( Duma, 2018 ). Furthermore, advanced breast cancer has a poor prognosis ( Wang et al, 2017 ). Although research on breast cancer pathogenesis has made great strides, including the tumor microenvironment (TME) and energy metabolism, the underlying pathogenesis of breast cancer must be clarified ( Zhou et al, 2019 ).…”
