Objectives This study aimed to evaluate the feasibility of reduced full-of-view synthetic high-b value diffusion-weighted images (rFOV-syDWIs) in the clinical application of cervical cancer based on image quality and diagnostic efficacy. Methods We retrospectively evaluated the data of 35 patients with cervical cancer and 35 healthy volunteers from May to November 2021. All patients and volunteers underwent rFOV-DWI scans, including a 13b-protocol: b = 0, 25, 50, 75, 100, 150, 200, 400, 600, 800, 1000, 1200, and 1500 s/mm2 and a 5b-protocol: b = 0, 100, 400, 800,1500 s/mm2. rFOV-syDWIs with b values of 1200 (rFOV-syDWIb=1200) and 1500 (rFOV-syDWIb=1500) were generated from two different multiple-b-value image datasets using a mono-exponential fitting algorithm. According to homoscedasticity and normality assessed by the Levene’s test and Shapiro–Wilk test, the inter-modality differences of quantitative measurements were, respectively, examined by Wilcoxon signed-rank test or paired t test and the inter-group differences of ADC values were examined by independent t test or Mann–Whitney U test. Results A higher inter-reader agreement between SNRs and CNRs was found in 13b-protocol and 5b-protocol rFOV-syDWIb=1200/1500 compared to 13b-protocol rFOV-sDWIb=1200/1500 (p < 0.05). AUC of 5b-protocol syADCmean,b=1200/1500 and syADCminimum,b=1200/1500 was equal or higher than that of 13b-protocol sADCmean,b=1200/1500 and sADCminimum,b=1200/1500. Conclusions rFOV-syDWIs provide better lesion clarity and higher image quality than rFOV-sDWIs. 5b-protocol rFOV-syDWIs shorten scan time, and synthetic ADCs offer reliable diagnosis value as scanned 13b-protocol DWIs.

BackgroundThe recommended technique for breast diffusion‐weighted imaging (DWI) acquisitions is not sufficiently standardized in clinical practice.PurposeTo investigate the intraobserver and interobserver reproducibility of DWI measurements, diffusion‐kurtosis imaging (DKI) parameters, and image quality evaluation in breast lesions between single‐shot echo‐planar imaging (ss‐EPI) and readout‐segmented echo‐planar imaging (rs‐EPI).Study TypeProspective.PopulationA total of 295 women with 209 malignant and 86 benign breast lesions.Field Strength/SequenceA 3‐T; fat‐saturated T2‐weighted MR imaging (T2WI); multi‐b‐value DWI with both ss‐EPI and rs‐EPI readouts; T1‐weighted dynamic contrast‐enhanced MRI (DCE‐MRI).AssessmentMean kurtosis (MK), mean diffusion (MD), and apparent diffusion coefficient (ADC) values were measured for each lesion on ss‐EPI and rs‐EPI, respectively. Image quality was visually evaluated regarding image sharpness, geometric distortion, lesion conspicuity, visualization of anatomic structures, and overall quality. Quantitative and qualitative analyses were performed twice with a time interval of 2 weeks.Statistical TestsIntraobserver and interobserver reproducibility were evaluated using intra‐class correlation coefficients (ICC), within‐subject coefficient of variation (wCV), and Bland–Altman plots.ResultsMK, MD, and ADC quantitative parameters for breast lesions showed excellent intraobserver and interobserver reproducibility, with ICCs >0.75 and wCV values ranging from 2.51% to 7.08% for both sequences. The wCV values in both intraobserver and interobserver measurements were higher in the ss‐EPI sequence (3.63%–7.08%) than that of the rs‐EPI sequence (2.51%–3.62%). The wCV values differed in subgroups with different histopathological types of lesions, breast density, lesion morphology, and lesion sizes, respectively. Furthermore, rs‐EPI (ICCs, 0.76–0.97; wCV values, 2.41%–6.04%) had better intraobserver and interobserver reproducibility than ss‐EPI (ICCs, 0.54–0.90; wCV values, 6.18%–13.69%) with regard to image quality.Data ConclusionCompared to the ss‐EPI, the rs‐EPI sequence showed higher intraobserver and interobserver reproducibility for quantitative diffusion‐related parameters and image quality assessments measured in breast DWI and DKI.Evidence Level2.Technical EfficacyStage 2.

Magnetic resonance imaging (MRI) is highly sensitive for lesion detection in the breasts. Sequences obtained with different settings can capture the specific characteristics of lesions. Such multi-parameter MRI information has been shown to improve radiologist performance in lesion classification, as well as improving the performance of artificial intelligence models in various tasks. However, obtaining multi-parameter MRI makes the examination costly in both financial and time perspectives, and there may be safety concerns for special populations, thus making acquisition of the full spectrum of MRI sequences less durable. In this study, different than naive input fusion or feature concatenation from existing MRI parameters, a novel Integrated MRI Multi-Parameter reinfOrcement fusion generatoR wiTh AtteNTion Network (IMPORTANT-Net) is developed to generate missing parameters. First, the parameter reconstruction module is used to encode and restore the existing MRI parameters to obtain the corresponding latent representation information at any scale level. Then the multi-parameter fusion with attention module enables the interaction of the encoded information from different parameters through a set of algorithmic strategies, and applies different weights to the information through the attention mechanism after information fusion to obtain refined representation information. Finally, a reinforcement fusion scheme embedded in a V − -shape generation module is used to combine the hierarchical representations to generate the missing MRI parameter. Results showed that our IMPORTANT-Net is capable of generating missing MRI parameters and outperforms comparable state-of-the-art networks.
