Direct Visual Inspection of the Cervix With Lugol Iodine for the Detection of Premalignant Lesions

Abstract: Direct visual inspection after Lugol iodine painting is feasible and easy to perform with superior sensitivity to cervical cytology and DVI-A in detecting cervical premalignant and malignant lesions. Direct visual inspection after Lugol iodine painting can be used as an efficient primary screening tool with a satisfactory low biopsy rate in low resources settings.

“…Moving the test of Lugol's staining (Schiller's test) from the "minor grade" category to the "nonspecific" category of the "abnormal colposcopic findings" section is because several studies such as those associated with the ASCUS-LSIL Triage Study showed poor reliability of Lugol's staining. 14,24,25 A cervical polyp is a common finding and has remained in the "miscellaneous findings," including its origin as being ectocervical or endocervical. …”

“…Most of the studies were from India (n = 11) [28][29][30][31][32][33][34] and Africa (n = 9), 28,[35][36][37][38] with just three studies from Oceania (Vanuatu) 39 and Latin America (Brazil and Columbia). 15,40 Nine different studies, conducted in four Indian and five African cities, were reported in one article.…”

“…15 Previous studies indicated that sensitivity of DVI in identification of pre-cancerous cervix lesions is more than Pap smear. 16 17 18 For example, a study in Tehran showed that, sensitivity of DVI was 96% and its specificity was 44% that were higher than sensitivity and specificity of Pap smear that were 42% and 10% respectively. 9 Another study in 2012, reported the sensitivity and specificity of DVI, 88.8% and 99.9% while they were 37.5% and 99.6% for Pap smear.…”

“… 20 Sensitivity of DVI in different studies was reported between 75 to 100% 13 14 15 17 and its specificity was reported between 16 to 85%. 16 17 18 21 Also, it is reported that sensitivity of DVI is higher than Pap smear while its specificity is lower than Pap test. 16 18 22 23 In our study sensitivity and specificity of DVI were 71.4% and 50% compared with 14.3% and 50% respectively for Pap smear.…”
