Evidence for Cognitive Decline in Chronic Pain: A Systematic Review and Meta-Analysis

Background: People with chronic pain (CP) sometimes report impaired cognitive function, including a deficit of attention, memory, executive planning, and information processing. However, the association between CP and cognitive decline was still not clear. Our study aimed to assess the association of CP as a risk factor with cognitive decline among adults.Methods: We included data from clinical studies. Publications were identified using a systematic search strategy from PubMed, Embase, and Cochrane Library databases from inception to October 10, 2020. We used the mean cognitive outcome data and the standard deviations from each group. The standardized mean difference (SMD) or odds ratio (OR), and 95% confidence intervals (CI) were performed for each cognitive decline outcome. I2-values were assessed to quantify the heterogeneities.Results: We included 37 studies with a total of 52,373 patients with CP and 80,434 healthy control participants. Because these studies used different evaluative methods, we analyzed these studies. The results showed CP was associated with cognitive decline when the short-form 36 health survey questionnaire (SF-36) mental component summary (SMD = −1.50, 95% CI = −2.19 to −0.81), the Montreal cognitive assessment (SMD = −1.11, 95% CI = −1.60 to −0.61), performance validity testing (SMD = 3.05, 95% CI = 1.74 to 4.37), or operation span (SMD = −1.83, 95% CI = −2.98 to −0.68) were used. However, we got opposite results when the studies using International Classification of Diseases and Related Health Problems classification (OR = 1.58, 95% CI = 0.97 to 2.56), the Mini-Mental State Examination (SMD = −0.42, 95% CI = −0.94 to 0.10; OR = 1.14, 95% CI = 0.91 to 1.42), and Repeatable Battery for the Assessment of Neuropsychological Status memory component (SMD = −0.06, 95% CI = −0.37 to 0.25).Conclusion: There may be an association between CP and the incidence of cognitive decline when some cognitive, evaluative methods were used, such as short-form 36 health survey questionnaire, Montreal cognitive assessment, performance validity testing, and operation span.

Background: One of the most common behavioral problems in patients with dementia is eating problems, which are known to increase the risk of malnutrition. However, few studies have been conducted in this patient group regarding the relationship between eating difficulties and nutritional status.Purpose: This study was designed to determine the eating difficulties faced by patients with dementia and to evaluate the relationship in this population between eating difficulties and malnutrition.Methods: This study was carried out in a dementia outpatient clinic of a university hospital in Istanbul, Turkey. This crosssectional, case-control study included 50 patients and 50 healthy controls as participants. Participants were assessed for eating difficulties and for nutritional, cognitive, and functional statuses. Results:The patients with dementia had more difficulties in terms of self-feeding skills. Although problems related to manipulating food on the plate and the use of utensils were not seen in the control group, these problems were found in 30% of the participants in the dementia group (p < .001). Moreover, 30% of the patients in the dementia group were unable to eat without assistance (p < .001). Associations were found between eating difficulties and age, duration of illness, and cognitive and functional (basic and instrumental activities of daily living) statuses. In addition, self-feeding skills were found to be associated with nutritional status. Rate of malnutrition or risk of malnutrition was higher in patients with dementia than in those in the control group.Conclusions: In this study, compared with the control group, patients with dementia had more problems in self-feeding skills such as manipulation of food on a plate, use of utensils, need for assistive tools, ability to eat without assistance, and negative eating behaviors (refusal to eat). An association was found between eating difficulties and nutritional status. Evaluating eating difficulties is recommended in patients with dementia to prevent nutritional deterioration.

AimsThe purpose of this study was to investigate the prevalence, influencing factors, and clinical characteristics of cognitive impairment in elderly patients with chronic schizophrenia.Materials and MethodsA total of 264 elderly patients with chronic schizophrenia and 156 normal controls were enrolled in the current study. The Mini-mental State Examination (MMSE) was used to assess their overall cognitive function, the Positive And Negative Syndrome Scale (PANSS) was used to assess their psychotic symptoms, the Geriatric Depression Scale (GDS) was used to assess their depressive symptoms, while Activity of Daily Living Scale (ADL) was used to assess their daily living ability.ResultsThe prevalence of cognitive impairment was 77.7% (205/264) in elderly patients with chronic schizophrenia, which was much higher than that [16.7% (26/156)] in normal controls. By using stepwise binary regression analysis, we found that hobbies (p < 0.001, OR = 0.224, 95% CI: 0.114–0.441) might be a protective factor for cognitive impairment, and this relationship remained statistically significant after adjusting for total scores of GDS, ADL and PANSS (modelb) (p = 0.016, OR = 0.406, 95% CI: 0.195–0.847). Compared with individuals without cognitive impairment, individuals with cognitive impairment tend to have more depression and psychiatric symptoms as well as worse activities of daily living (p < 0.05). Through linear regression analysis of the mediating model, we found that hobbies may improve cognitive function by improving psychiatric symptoms, and play a partial mediating role (B = −4.789, p < 0.001).ConclusionCognitive impairment is a very prominent problem in elderly patients with chronic schizophrenia. Elderly schizophrenia patients with cognitive impairment tended to have more depressive mood, more psychotic symptoms and worse activities of daily living. Hobbies will help prevent cognitive impairment in elderly patients with schizophrenia and may improve their cognitive function by influencing psychiatric symptoms. Therefore, we should encourage elderly patients with chronic schizophrenia to develop their own hobbies. However, the above conclusion still need to be further verified, as we cannot exclude the effects of age and education.
