Dislocation of the hip in cerebral palsy. Natural history and predictability

Abstract: To determine the natural history of dislocation of the hip in cerebral palsy, and to evolve methods to predict dislocation, the notes and radiographs of 462 patients with cerebral palsy were reviewed. Dislocation occurred in 10% of patients by progressive migration and subluxation of the proximal femur in the presence of acetabular dysplasia. Statistical analysis identified the radiographic features that helped to predict dislocation. Measurement of acetabular index, by a method that allows for rotation of the… Show more

“…A study of children with cerebral palsy demonstrated acetabular dysplasia must be present for dislocation to occur, even if increased muscle tone was present [6]. Therefore initial treatment with the Pavlik harness allowing resolution of dysplasia could result in the successful development of the acetabulum and consequently prevent further hip dysfunction.…”

“…Literature review and determination of probable events based on consensus building A literature review was performed with a focus on the following: the natural history of unstable hips in CP, [9][10][11][12] operative complications of FVO 13 and delayed hip reconstruction including pelvic osteotomy, [14][15][16] contralateral hip instability after unilateral FVO in individuals with CP, 1,5,17,18 and pain utility measures in the hips. 19 Each event was reviewed on the basis of probability.…”
