Disseminated intravascular coagulation associated organ failure in obstetric patients admitted to intensive care units: a multicenter study in China

Abstract: We aimed to determine disseminated intravascular coagulation (DIC)-associated organ failure and underlying diseases based on data from three ICU wards in tertiary hospitals in China from 2008 to 2016. The diagnosis of DIC was confirmed by an International Society of Thrombosis and Hemostasis score greater than or equal to 5. The maternal outcomes included the changes in organ function 24 h after ICU admission. The durations of hospital stay and ICU stay were recorded as secondary outcomes. Among 297 ICU admiss… Show more

“…Elevated creatinine levels were detected in 14 patients (higher than the reference value of 1.2), and ARF developed in 4 (7%) patients and were referred to multidisciplinary centers. In a study by Zhao et al, ARF (16.5%) was stated as the most common type of organ failure (19). The overall ARF rate in our study (7%, 4/57) was lower than in other studies (24.1% to 61%) in developing countries (20,21).…”

“…Disseminated intravascular coagulation is an independent and most important predictor of multi-organ failure especially kidney, in critically ill obstetrics. 4 The main management of DIC consists of treating the underlying cause and administering blood, blood components, and fibrinogen concentrate according to ROTEM. But interventional management like transarterial embolization (TAE) and percutaneous drainage are most effective in controlling the specific bleeders responsible for hematoma and hemoperitonium.…”

“…DIC always occurs as a secondary disorder and is commonly associated with postpartum hemorrhage, followed by hypertensive disorders, acute fatty liver of pregnancy, sepsis, and amniotic fluid embolism (AFE). [2] In clinical obstetrics, DIC often occurs during delivery or postpartum, while cases of prenatal acute DIC as the first manifestation are uncommon. Patients with DIC have higher rates of multiple organ dysfunction syndrome, including acute renal failure (ARF), respiratory failure, and disturbance of consciousness.…”
