Doença de Hirschsprung – Dismotilidade intestinal pós‐cirúrgica

Abstract: Hirschsprung's disease presenting as total colonic aganglionosis has clinical and surgical characteristics that differentiate it from the classic forms, complicating the diagnosis and the clinical and surgical management. The postoperative course may be associated with permanent morbidity due to intestinal dysmotility. The numerical reduction or alteration of neural connections in the interstitial cells of Cajal may represent a possible physiopathological basis for the condition.

“…It has been proposed that the fetus may be affected by inflammation or hypoxic-ischaemic injury antenatally leading to ICC, ICC progenitor and enteric nerve damage 6. An alternative hypothesis is that ICC may be susceptible to ischaemic damage during periods of obstruction or that the reduction in ICC numbers per area of bowel may be secondary to thinning of the bowel due to chronic obstruction 7…”

“…Several studies have examined the association of HD and/or hypoganglionosis with reduced ICC in literature,6–10 but to date, no definitive association has been proven. The combined impairment of ICC in the aganglionic and ganglionic parts of the bowel in HD might be responsible for the frequently observed motility disorders that persist after successful resection in HD 11.…”
