Does a regular Wessex Head Injury Matrix assessment identify early signs of infections in people with Prolonged Disorders of Consciousness?

Abstract: The findings suggest the importance of serial WHIM assessments throughout the period of recovery, not only to measure cognitive changes but also to highlight underlying physical changes such as infections that will impact the response to rehabilitation and recovery.

“…Dhamapurkar and colleagues [ 56 ] reported that regular WHIM assessments might identify early signs of infections in PDOC patients. They correlated this scale scores with the pre-infection period and with post-infection and observed that the decrease in WHIM scores was related to the infection insurgence.…”

“…Dhamapurkar, Wilson, Rose, and Florschutz (2015b) looked at patients with a DOC lasting 12 months or more to see if recovery was more likely for those who had sustained a TBI compared to those whose DOC resulted from other causes (mainly hypoxia). All patients (28 total) with a DOC admitted to a rehabilitation center over a 3-year period were assessed with the WHIM and the Disability Rating Scale (DRS; Hall, Hamilton, Gordon, & Zasler, 1993) to determine if they had emerged from a DOC.…”

“…Shiel [6] and Majerus [13] tested the inter-rater reliability (IRR) and test-retest reliability (TRR) of the WHIM on a group of 25 and 5 subjects, respectively. Other studies highlighted the potentiality of WHIM in assessing patients with prolonged DOC (PDOC) [14,15].…”
