Does laser ablation prostatectomy lead to oncological compromise?

Abstract: records, histopathology databases and The British Association of Urological Surgeons Cancer Registry. We recorded the total number of prostate cancers diagnosed per year, number of TURPs performed, Gleason scores and patients outcomes. Of newly diagnosed patients, 82% were allocated to active surveillance, whilst 18% were started on hormone therapy, with no prostate cancer-related deaths over a mean ( SEM , range) follow-up of 49.7 (2.4, 11-81) months. RESULTS TURP CONCLUSIONSTURP mainly samples transitional-z… Show more

“…This figure remained significant on subgroup analysis in men aged ≤ 65 years at 13.4%. These figures of ICP on all TURP specimens are higher when compared with previous literature, with reported rates of 4.1–16.7%, 5 , 10 , 18 , 19 , 20 , 21 , 22 summarized in Table 5 . When patients with suspicious rectal examination, high PSA, or previous positive biopsy are excluded, ICP occurs less frequently at 1.8–5.5%.…”

“…Most prostate cancers arise from the PZ of the prostate, with recent studies suggesting that isolated TZ tumors occur in only 2–7% of cancers. 4 , 5 , 6 In addition, TZ tumors tend to correspond with a more favorable oncological outcome, including lower Gleason scores, rates of Extra-prostatic extension (EPE), and risk of biochemical recurrence. 5 Intuitively, TURP tends to result in the resection of the TZ, while the PZ is sampled the least with this treatment.…”

“…Postoperative histopathology revealed that two of our patients (0.9%) with normal preoperative PSA levels had prostate cancer. Although initial results indicate that the rate of detection of incidental prostate cancer in PVRP seems lower than in TURP or prostate enucleation [17,18], this technique overcomes the problem of the lack of postoperative tissue samples in PVP. We believe that as experience in using PVRP increases, a higher rate of incidental prostate cancer detection can be expected.…”

“…[4][5][6] Several recent studies have reported that cancer arising from the TZ have a more favorable prognosis than Thapa N. et al Incidence of carcinoma prostate in transurethral resection specimen jlmc.edu.np tumors that arise in the peripheral zone (PZ). 4 As a result, several groups argue that the trans-urethral resection of prostate (TURP) specimen may hold limited diagnostic value.…”
