Introduction: Benign prostate hyperplasia (BPH) is a common problem in elderly men. The current gold standard is surgical transurethral resection of the prostate (TURP). Prostatic artery embolization (PAE) is an alternative treatment of BPH, which avoids surgical complications. This is a single-center prospective study from the Kingdom of Bahrain to evaluate the effectiveness of PAE.Methods: This prospective, single-center study included consecutive patients eligible for PAE. Patients were evaluated at one, three, six, and 12 months. Clinical success was defined as a decrease of International Prostate Symptom Score (IPSS) ≥ 3 points, quality of life (QoL) score ≤ 3 or decrease by three points, and no need for surgical intervention. Correlation between prostate-specific antigen (PSA) at 24 hours and reduction in prostate volume and IPSS score was also assessed.Results: A total of 29 patients underwent the procedure between June 2015 and August 2018. Bilateral embolization was achieved in 26 patients. Clinical success was achieved in 26 patients (89.65%). No major adverse events were encountered. Significant improvement for storage (−4.54 ± 2.93, p < 0.005) and voiding symptoms score (−7.54 ± 4.74, p <0.005) were seen. There was no significant correlation between 24 hours PSA and reduction of the size of prostate and IPSS score at 12 months. Conclusion: PAE is a minimally invasive procedure that is safe and effective for the management of BPH. PAE is effective for the management of both storage and voiding symptoms.

Purpose To compare the relative IPSS (International Prostate Symptom Score) improvement in storage and voiding symptoms between prostatic artery embolization (PAE) and transurethral resection of the prostate (TURP). Method Retrospective analysis of the UK-ROPE (UK Register of Prostate Embolization) multicentre database was conducted with inclusion of all patients with full IPSS questionnaire score data. The voiding and storage subscore improvement was compared between interventions. Student’s t-test (paired and unpaired) and ANOVA (Analysis of variance) were used to identify significant differences between the groups. Results 146 patients (121 PAE, 25 TURP) were included in the analysis. Storage symptoms were more frequently the most severe symptom (‘storage’ in 75 patients vs ‘voiding’ in 17 patients). Between groups, no significant difference was seen in raw storage subscore improvement (TURP 4.9 vs PAE 4.2; p = 0.34) or voiding subscore improvement (TURP 8.4 vs PAE 6.7; p = 0.1). ANOVA demonstrated a greater proportionate reduction (relative to total IPSS) towards voiding symptoms in the TURP group (27.3% TURP vs 9.9% PAE, p = 0.001). Conclusion Although both TURP and PAE improve voiding symptoms more than storage, a significantly larger proportion of total symptom reduction is due to voiding in the TURP cohort, with PAE providing a more balanced improvement between voiding and storage.

ContexteL'hypertrophie bénigne de la prostate (HBP) est la maladie la plus fréquente chez les hommes. A l' échelle de la Suisse, elle touche environ 40% des hommes de plus de 50 ans, et même environ 80% des hommes de plus de 80 ans. L'HBP se manifeste par des problèmes mictionnels. Les symptômes du bas appareil urinaire (SBAU) typiques incluent un jet urinaire affaibli avec une miction retardée et prolongée, l'impériosité mictionnelle, la pollakiurie et la nycturie, ainsi que les gouttes retardataires après la vidange de la vessie.Le traitement initial de l'HBP symptomatique est médicamenteux, faisant typiquement appel aux antagonistes des récepteurs α-1, aux inhibiteurs de la 5-α-réductase ou à une combinaison des deux.
