ObjectiveTo investigate the role of inflammatory markers, including neutrophil to lymphocyte ratio (NLR), platelet to lymphocyte ratio (PLR), and monocyte to lymphocyte ratio (MLR), c‐reactive protein (CRP) to albumin ratio (CAR), fibrinogen to albumin ratio (FAR), and fibrinogen to CRP ratio (FCR) in predicting the latency period (≤72 vs. >72 hours) before preterm birth.Materials and MethodsIn a retrospective study, we assessed 135 patients meeting the specified criteria with signs of preterm labor (<34 weeks). The patients were categorized into two groups: 71 patients giving birth within 72 h (latency ≤ 72 h) and 64 patients giving birth after 72 h (latency > 72 h). We examined the demographic and medical characteristics and perinatal outcomes of all participants. Categorical variables between groups were compared using the Chi‐square test. The Student's t‐test was utilized for normally distributed continuous variables, and the Mann–Whitney U test was applied for non‐normally distributed data. Receiver operating characteristic (ROC) curve analysis was conducted to identify the optimal cut‐off levels for inflammatory markers in predicting the latency period before birth.ResultsAmong the parameters examined, significant differences were observed between the groups only in terms of CAR and FCR. While CAR showed a significantly higher value in the group with latency period ≤72 h (0.537 ± 1.239 vs. 0.247 ± 0.325, p = 0.022), FCR showed a significantly lower value in the group with latency period ≤72 h (63.58 (2.99–1165) vs. 88.93 (9.35–1165), p = 0.013). The identified cut‐off value for CAR was 0.190, providing a sensitivity of 57.7% and a specificity of 56.3% (p = 0.022). The cut‐off value for FCR was 71.67, with a sensitivity of 42.3% and a specificity of 42.2% (p = 0.013).ConclusionsThe CAR and the FCR, serving as predictive markers for preterm labor, may offer a simple, cost‐effective, and easily accessible approach, particularly in resource‐limited settings.

Fibrinogen to albumin ratio, a novel serum indicator for evaluating the severity of preeclampsia: A single-center retrospective study

Preeclampsia (PE) is a disorder that affects approximately 5% to 10% of pregnant women. Timely and accurate identification of PE and assessment of its severity are crucial. Therefore, it is necessary to develop predictive indicators which are easily measured in routine antenatal examinations to enable the early detection of PE and assess its severity. We designed a single-center retrospective study in our daily work to assess whether the serum levels of fibrinogen to albumin ratio (FAR), fibrinogen (Fib), albumin (ALB), prothrombin time, calcium (Ca), activated partial thrombin time, creatinine (Cr), D-dimer(D-D), platelet, white blood cell, neutrophil, and lymphocyte counts could help in assessing PE and evaluating its severity. Our findings showed that the serum levels of FAR, Cr, Fib, and D-D were significantly higher in the severe preeclampsia group (sPE) compared with the control and mild preeclampsia groups, whereas the levels of ALB and Ca were significantly lower in sPE patients. In addition, no differences were found between the control and PE groups in terms of prothrombin time, activated partial thrombin time, platelet, white blood cell, neutrophils, and lymphocytes counts. Furthermore, FAR is a novel and better indicator for evaluating the severity of PE, which has not been reported before. And it is an independent risk factor for the development of sPE. In conclusion, the serum levels of FAR, Cr, D-D and Fib were positively correlated with PE, whereas ALB and Ca were negatively correlated with PE severity, which might be valuable in evaluating the severity of PE. FAR proved to be a feasible diagnostic marker for sPE with sensitivity and specificity comparable to those of ALB and Fib.

Latar belakang : Abortus Imminens adalah peristiwa terjadinya perdarahan dari uterus pada kehamilan sebelum 20 minggu, saat hasil konsepsi masih dalam uterus tanpa adanya dilatasi serviks. Abortus yang sedang mengancam atau berlangsung, seringkali membuat banyak wanita mengalami stres karena tidak mengetahui hal yang akan terjadi. Janin dalam Abortus Imminens masih dapat dipertahankan (pada 95% kasus) dengan istirahat total atau tirah baring dan 5% kasus kehamilan menjadi lebih singkat dan bayi lahir prematur. Tujuan : Penelitian ini bertujuan untuk mengetahui faktor-faktor penyebab kejadian Abortus Imminens di RSIA Putra Dalima BSD Tahun 2019. Metode : Penelitian ini bersifat analitik observasional dengan desain kasus dan pengumpulan data diperoleh dari rekam medik rumah sakit. Populasi dalam penelitian ini adalah seluruh ibu hamil yang dirawat sebanyak 95 pasien ibu hamil dengan 41 kasus yang mengalami Abortus Imminens dan sebanyak 54 ibu hamil yang tidak mengalami Abortus Imminens. Hasil : Hasil penelitian menjukan bahwa kejadian Abortus Imminens di Rumah Sakit Ibu dan Anak Putra Dalima sebanyak 41 kasus (43%). Usia ibu, usia kehamilan, paritas, dan riwayat keguguran sebelumnya berpengaruh terhadap kejadian Abortus Imminens. Faktor penyebab yang paling dominan adalah usia ibu dan usia kehamilan. Dengan faktor-faktor tersebut kejadian Abortus Imminens dapat mengkonsumsi obat Allystrenol, Isoxsuprine, Asam Folat sesuai dengan petunjuk dokter.
