Dosimetric evaluation of different planning techniques based on flattening filter-free beams for central and peripheral lung stereotactic body radiotherapy

Abstract: This study aimed to dosimetrically compare and evaluate the flattening filter-free (FFF) photon beambased three-dimensional conformal radiotherapy (3DCRT), intensity-modulated radiation therapy (IMRT), and volumetric modulated arc therapy (VMAT) for lung stereotactic body radiotherapy (SBRT). RANDO phantom computed tomography (CT) images were used for treatment planning. Gross tumor volumes (GTVs) were delineated in the central and peripheral lung locations. Planning target volumes (PTVs) was determined by add… Show more

“…It's crucial to note that the performance of both DMLC and VMAT plans is heavily reliant on the optimization functions and the speci c characteristics of the tumor and surrounding normal tissues. As a result, it's feasible that DMLC could outperform VMAT in terms of conformity in certain instances, despite VMAT generally being presumed to offer better or at least equal conformity [23][24][25] . Overall, both VMAT and DMLC plans can achieve comparable dose distributions in SBRT.…”

“…In the case of (FFF) VMAT plans, the treatment delivery time is primarily constrained by the rotational speed of the gantry, rather than the dose rate. Accelerated delivery may potentially decrease the likelihood of intra-fractional setup inaccuracies, which have been reported to occur in treatment procedures lasting over 15 minutes [7,12].…”

“…The phantom is a 175 cm, 73.5 kg male body sliced at 2.5 cm intervals. [6,7] Simulation: A CT scan of 25mm slices of the phantom is performed to create a 3D image of the phantom. This image is used to develop a treatment plan, similar to what would be done for a patient.…”

“…Recently, several reports have examined various VMAT techniques for treating lung cancer, analyzing their impact on normal lung dose and confirming their feasibility. 14 , 15 , 16 One of those studies investigated the benefit of the fixed jaw and jaw tracking VMAT technique, one study evaluated the benefit of the flattened filter‐free photon beam, and one study added a non‐coplanar arc to reduce dose to organs at risk (OAR). Nevertheless, to our knowledge, nearly none of the studies investigated the dosimetric effect of dual‐isocenter jaw‐tracking VMAT in lung cancer with a large longitudinal tumor target.…”
