Double-outlet left atrium: Ventriculo-atrial malalignment defect

Abstract: A four-month-old female infant presented with heart failure and cyanosis. On echocardiography, there was no outlet to the right atrium except an atrial septal defect and the left atrium drained into the respective ventricles by two atrioventricular valves. The case is reported for its rarity, with a brief literature review.

“…Additionally, the right AV valve straddles the muscular ventricular septum over a VSD, allowing for ejection of blood into both the RV and LV, although the LV is often hypoplastic in this scenario. While this represents another form of DORA, 7 , 12 it is anatomically distinct from the first scenario described. The cat of the present case was consistent with the former DORA scenario in combination with a transitional AVSD.…”

“…The very presence of multiple attempts at classification [Tables 2 and 3 ] shows that distinction of the varied forms of “double outlet atrium” remains far from clear. [ 29 31 ] The feature that differentiates the solitary junction from the common junction is the location and attachment of the leading edge of the atrial septum. If the leading edge of the atrial septum fails to reach the parietal part of the atrioventricular junction then there must be an ostium primum defect, and by definition, there will be a common atrioventricular junction.…”

“…Double-outlet left atrium (DOLA) is an extremely rare anomaly; a recent report found that only seven cases had been described so far. [ 1 ] To our knowledge, there have been only two published reports of repair of DOLA. [ 2 3 ] From the Indian subcontinent, only two such cases have been reported, neither of which was operated upon for different reasons.…”

“…[ 2 3 ] From the Indian subcontinent, only two such cases have been reported, neither of which was operated upon for different reasons. [ 1 4 ] We here describe the clinical detail, echocardiographic features, angiographic evaluation, and surgical repair of DOLA with intact ventricular septum and common atrioventricular (AV) valve in a 2-year-old boy.…”
