Double Root Anatomical Variations in a Single Patient: Endodontic Treatment and Rehabilitation of a Three-rooted First Premolar. Case Report

Abstract: SUMMARY:Abnormal interaction between oral epithelium and underlying mesenchematic tissue during odontogenesis could result in teeth with an anatomical variation. First maxilar premolar has such variable root canal morphology. However, (0.2-6% of cases) a three-rooted premolar is particularly uncommon. First mandibular premolar frequently has one root and one single canal (75% of cases), however, a tooth with two canals and two separate roots could be found (19.5-22.5%). Execution of an accurate diagnosis of ro… Show more

“…Therefore, in dental practice, a detailed radiographic analysis is necessary, especially prior to performing procedures within the RCS [14,15,17]. Existing results are consistent with this statement, where size and volume of molars crowns did not lead to presume any alteration in roots form (Fig.…”

“…Root anatomical variations are not always associated to a variation in the tooth crown that suggests an altered root form [15]. Therefore, in dental practice, a detailed radiographic analysis is necessary, especially prior to performing procedures within the RCS [14,15,17].…”

“…As imagens radiográficas e a própria análise radiográfica nem sempre são dados suficientes para obter uma abstração adequada do sistema de canais radiculares. A forma e configurações do sistema de canal é sempre variável (Oporto et al, 2013). A tomografia não deve ser usada para avaliação de doença periapical antes do tratamento endodôntico.…”

“…Root canal anatomy and the macro-morphology of any tooth is not a constant factor, variations can be presented and the clinicians should be acquainted with it to achieve the goal of endodontics, prevention and elimination of apical periodontitis. 1 Lack of adequate knowledge of these variations result in iatrogenic mishap of missed anatomy and can predispose to treatment failures resulting in orthograde and in some cases retrograde endodontic retreatment. 2 The root canal and the macro-morphology of maxillary premolars are highly variable.…”
