Downregulation of genes related to immune and inflammatory response in IVF implantation failure cases under controlled ovarian stimulation

Abstract: Importance of immune and inflammatory response in endometrial receptivity adds on to the current knowledge of gene expression profile in IF under COS. The panel of genes involved in these pathways would be useful in determining further line of treatment for IF during IVF.

“…LIF was proposed as a biomarker for endometrial receptivity, since it is a pleiotropic cytokine playing multiple roles in endometrium and embryo differentiation. 32 Interestingly, it has been shown that urine-purified and recombinant hCG are able to stimulate the production of LIF both in vitro 33 and human female in vivo. 31 In humans, endometrial expression of LIF was significantly decreased in patients with implantation failure as compared with healthy donors.…”

“…However, uterine receptivity is deteriorated during COH in animal and clinical study. [6][7][8] Controlled ovarian hyperstimulation reduces VEGF during uterine receptivity, 4 which is regulated by estrogen and selective ER modulators in endometrial stromal cells. 9 It has been suggested that, due to COH, the peak serum estradiol level during the early luteal phase caused a time shift of the implantation window.…”

“…Therefore, the key for increasing embryo implantation rate and clinical pregnancy rate is to improve endometrial receptivity. Multiple studies have shown that leukemia inhibitory factor (LIF), insulin-like growth factor-I (IGF-1), and epidermal growth factor (EGF) exhibit high expression levels during the implantation window of several different species of mammals [8,9]. In addition, the concerted effects of these factors contribute to the establishment of endometrial tolerance.…”
