Drug Induced Steven- Johnson Syndrome (SJS)

Abstract: Steven Johnson Syndrome (SJS) or Toxic Epidermal Necrolysis (TEN) is a life threatening immune complex mediated hypersensitivity reaction which mainly involves skin and mucous membrane. SJS is generally induced by the drugs. The manifestations include blisters on skin, facial swelling and hyper pigmentation. Here is a 28-yearold Indian male received Ofloxacin, Paracetamol and Phenobarbitone and experienced a severe skin reaction which was diagnosed as Stevens Johnson syndrome (SJS). The above drugs will be imp… Show more

“…Adverse drug reactions (ADRs) are estimated as the fourth to sixth leading cause of death (Dormann et al, 2000;Pouyanne et al, 2000;Miya et al, 2019). While the majority are classified as type A, predictable based on drug pharmacology, the remainder are off-target type B ADRs and inclusive of T-cell-mediated delayed drug hypersensitivity reactions (DHRs).…”

“…SJS and TEN are the same disease across a spectrum of severity with the higher end of mortality (TEN) associated with up to 50% death (Patel et al, 2013;Langley et al, 2018). SJS/TEN is characterized by blistering and involvement of at least two mucous membranes (Paulmann and Mockenhaupt, 2015;Miya et al, 2019;Zimmerman and Dang, 2020). Despite clinical distinction, lack of mechanistic delineation has precluded development of disease-specific treatment and prevention strategies (Pavlos et al, 2015;Redwood et al, 2018).…”

“…Chloroquine is also a commonly prescribed drug for malaria, rheumatoid arthritis, and amoebic liver abscess. Both the drugs are considered as comparatively safer, but undesirable, untoward adverse effects mainly cutaneous hypersensitivity reactions have been reported in only few instances due to these drugs [3,4].…”

“…It accounts 6% of the total hospital admission and it leads to the economic burden to patient and health care system. 1 Among the various ADR cutaneous drug reactions mainly Stevens Johnson syndrome (SJS) is a rare immune-mediated adverse reaction it is the result of extensive death of keratinocytes, which leads to the separation of areas of skin in the dermal-epidermal junction. The incidence rate of SJS is 0.05 to 2 persons per million populations per year.…”

“…2 The most common drugs which cause SJS are sulphonamides, non-steroidal anti-inflammatory drugs, imidazole antifungal, cephalosporins, anticonvulsants, allopurinol, broad-spectrum bactericidal agents and highly active antiretroviral therapy highly active antiretroviral therapy (HAART) regimen. 3 Sulfadiazine is the antibiotic it is mainly used in combination with pyrimethamine to treat the toxoplasmosis it inhibits the tetrahydrofolic acid formation from para-amino benzoic acid, preventing the formation of folic acid in bacterial cell. It mainly acts on the gram-positive bacteria.…”
