Drugs in Pregnancy and Lactation – Medications Used in Swiss Obstetrics

Abstract: The same active constituents of medications are consistently used in Swiss perinatal centres for the main indications in pregnant women. Despite the existing experience and available evidence, they are mainly used off-label. Official authorisations for frequently or consistently used active ingredients should be granted.

“…An assessment of the internal treatment recommendations in the main Swiss obstetrics centres showed that B. pinnatum preparations are being prescribed for the treatment of preterm contractions [21]. This is in line with a Swiss online survey which showed that in approximately ¾ of the cases B. pinnatum preparations are administered in combination with synthetic tocolytics [22].…”

“…In the last decade, B. pinnatum began to be used at university institutions in Switzerland, where at present it constitutes an inherent part of the standard preterm contractions treatment (see [15]). The relatively low evidence level on which this use of B. pinnatum is based, together with the lack of fully efficacious and well-tolerated synthetic, tocolytic agents, makes the performance of randomised controlled trials urgent.…”

“…In view of the present paucity of well‐tolerated and long‐term unproblematic treatments for RLS, our observations suggest that it might be worthwhile to treat RLS patients with (add‐on) BP preparations. These might be particularly indicated in the treatment of (frequent) RLS during pregnancy—especially if iron substitution is not needed or not enough—since the standard RLS medications are contra‐indicated in pregnancy and BP preparations are already recommended and in use for other indications . In a recent prospective observational study performed to assess BP prescriptions in the field of gynecology and obstetrics in Switzerland, one case showing a very good effect of BP medication on RLS during pregnancy was described .…”
