Dupuytren’s disease: where do we stand?

Abstract: Dupuytren’s disease is a fibroproliferative disease that involves collagen deposition, leading to hand contractures that ultimately affect hand mobility and grip strength. It is a benign disorder but can cause high morbidity by limiting daily activities. Many factors have been proposed for its aetiology: namely genetics, smoking, alcohol intake and diabetes. However, there is still controversy as to the main aetiological cause of the disease. Treatment is not yet uniform around the world and still varies with … Show more

“…Inhibition of MLCK and CaM by specific inhibitors (ML-7 and W-7, respectively), prevents the migration of myofibroblasts induced by cardiotrophin-1 participating in the repair of scars after infarction [68]. Dupuytren's disease is an idiopathic fibroproliferative condition characterized by the formation of nodular lesions in the palmar fascia where the accumulation of fibroblasts and highly contractile myofibroblasts severely alters the function of the hands (reviewed in [102]). The CaM antagonist fluphenazine inhibited the migration of fibroblasts isolated from the collagen lattice of pathological lesions of patients with Dupuytren's disease, and this was attributed to MLCK inhibition [62].…”

“…Furthermore, according to the subgroup’s analysis of the underlying diseases, the highest prevalence of the disease, after diabetic patients, is related to alcoholic patients (24%). Various studies reveal that the prevalence of Dupuytren in alcoholics is higher than the general population; however, the cause is still not clear [ 79 , 80 ]. Alcohol consumption and its misuse as a social harm have complex interconnected economic, social and cultural causes and have serious safety and health consequences for the society.…”

“…Verglichen mit den hohen Rekurrenzraten der NF sind diese bei der limitierten Fasziektomie (lF) sowie bei der Dermofasziektomie (DF) deutlich niedriger (lF mit 6-21 %, DF 0 %) [12]. Durch ihre deutlich höhere Invasivität mit der Notwendigkeit eines Skingrafts bei der DF sollte diese Op tion jedoch eher erst im fortgeschrittenen Stadium erwo gen werden [5].…”
