With the widespread clinical use of prostate‐specific antigen (PSA), biopsy of the prostate has become one of the most commonly performed urological procedures. In general it is well tolerated, although there is some morbidity and risk of infection. In recent years, there have been increasing concerns that prostate biopsy may lead to tumour seeding along the needle tract. The aim of the present paper was to review the evidence on the prevalence of tumour seeding after prostate biopsy and to define the risk of this event in the context of current clinical practice. A PubMed literature search was conducted in January 2014 according to the Preferred Reporting Items for Systematic Reviews and Meta‐analysis (PRISMA) statement. Literature was examined with emphasis on the incidence of seeding, clinical presentation and on risk factors including type of needle used, transrectal vs transperineal approach, as well as tumour grade and stage. In all, 26 publications were identified reporting needle‐tract seeding after prostate biopsy. In all, 42 patients with needle‐tract seeding were identified. In most cases, seeding was reported after transperineal biopsy of the prostate, while nine cases occurred after transrectal biopsy. Based on the reviewed series the incidence of seeding appears to be <1%. The increase in the number of biopsies and cores taken at each biopsy over the years has not resulted in an increase in the reported cases of seeding. In conclusion, seeding along the needle track is a rare complication after prostate biopsy. Its actual incidence is presently difficult to quantify. It is reasonable to advise appropriate counselling and take measures to reduce this event where possible; however, we do not advocate avoidance of biopsies as the benefits of appropriate cancer diagnosis and management outweigh any potential risks from seeding.

354 Prostatakarzinom-Kranke wurden nach diagnostischen Merkmalen und therapeutischen Ergebnissen analysiert. Bei zo Fällen war eine Verlaufskontrolle möglich. Beste Behandlungserfolge brachte die Östrogentherapie in Kombination mit Orchiektomie: in 47,4 % der Fälle konnte eine Fünfjahres-CTberlebenszeit erzielt werden. Besondere ätiologische Faktoren ließen sich nicht feststellen. Die histologische Sicherung durch Biopsie erscheint ebenso wichtig wie die konsequente Hormonbehandlung bis zum Lebens ende der Patienten. Eine Frühdiagnose ist nur dann möglich, wenn jeder Mann über 40 Jahre einmal im Jahr rektal untersucht wird. Das Prostatakarzinom hatte früher wie andere maligne Geschwülste einen deletären Verlauf. Erst die mit dem Nobelpreis 1966 honorierten Untersuchungen von Huggins () über die Wirkung gegengeschlechtlicher Hormone brachten die entscheidende Wende. Wenn auch eine Heilung nicht möglich ist, kann selbst noch in schweren Fällen durch Ostrogene eine Besserung und Iebensverlängernde Wirkung erzielt werden. Diese Therapie gehört heute zu den Standardmethoden der Urologie. Ziel unserer Untersuchungen var es, symptomatisch-diagnostische Merkmale und therapeutische Ergebnisse aus einer langjährigen Beobachtungszeit zusammenzustellen und den Wert der Methode am eigenen Krankengut zu überprüfen. Die allgemeine Kenntnis von Problematik, Diagnostik und Therapie beim Prostatakarzinom setzen wir voraus. Patienten und BehandlungsschemaUnsere Befunde stützen sich auf Patienten mit Prostatakarzinomen. Bei zo Fällen war eine Verlaufskontrolle möglich. Die Patienten selbst, deren Angehörige oder die behandelnden Ärzte wurden aufgesucht, sofern die Patienten nicht in eigener poliklinischer Behandlung standen.Die Therapie erfolgte in den letzten Jahren entsprechend einem bei uns üblichen Schema. 7lo g Diäthylstilböstrol (Honvan®) wurden initial intravenös verabreicht. Im gleichen Zeitraum erfolgte die Orchiektomie. Die Östrogen-Dauertherapie wurde intramuskulär mit Depotpräparaten wie Progynon® (Östradiol) oder Cyren®B (Diäthylstilb5strol), durch Implantation con Cyren®A oder oral mit Progynon®M (Äthinylöstradiol) bzw. in den letzten drei Jahren mit Merbentul® (Chlortrianisen) durchgeführt. In the eighth decade of life cancer of the prostate is the leading cause of cancer death in men.16 In over 70% of autopsied cases, metastases have involved soft tissues and bones, in particular the pelvis, sacrum and lower vertebral column.4 The roentgenographic appearance is purely osteoblastic in over 78% of the cases8 and by its multiplicity, location and clinical background usually poses no diagnostic problem. Occasionally, a solitary metastasis in an unusual bone site is the sole manifestation of prostatic carcinoma and precedes the urologic symptoms by as much as two years.14 Exceptionally, a remotely located solitary metastasis of a clinically occult prostatic cancer may mimic a primary osteogenic sarcoma by the radiological appearance of sunburst calcification, as was the case in the present report.
