Early human experience with per-oral endoscopic pyloromyotomy (POP)

Abstract: POP is a technically safe and feasible endoscopic procedure. Early follow-up suggests promising symptomatic improvement as well as objective improvement in gastric emptying. Additional clinical experience is required to establish the role of this technique in the management of gastroparesis.

“…There are also recent case reports of endoscopic pyloromyotomy, performed through a gastric submucosal tunnel similar to the peroral endoscopic myotomy for achalasia, to provide symptom relief in gastroparesis [46][47][48]. Although these establish feasibility, longer follow-up in larger series and, if possible, comparative or controlled trial data are needed before implementation in clinical practice at larger scale.…”

“…Subsequently, case reports and small single-center series showed encouraging results with G-POEM. [10][11][12] Here, we aim to report the first multicenter experience with G-POEM and assess the efficacy and safety of this novel procedure for gastroparetic patients with symptoms refractory to medical therapy.…”

“…The standard treatment is balloon dilation, pyloroplasty, or occasionally surgical distal gastrectomy [1]. Peroral endoscopic pyloromyotomy (POP) has been used to improve gastric emptying in selected patients with refractory gastroparesis due to diabetes [2,3] or with gastric outlet obstruction following esophagectomy [4]. To the best of our knowledge, this is the first case in which POP was used to treat PPS.…”
