Early rehospitalization after initial chronic kidney disease educational hospitalization relates with a multidisciplinary medical team

Abstract: BackgroundIt is well-documented that chronic kidney disease (CKD) often results in end-stage renal failure and puts patients at extremely high risk for developing cardiovascular disease.
