Early report of a randomized comparative clinical trial of Strattice™ reconstructive tissue matrix to lightweight synthetic mesh in the repair of inguinal hernias

Abstract: Strattice™ is safe and effective in repairing inguinal hernia, with comparable intra-operative and early postoperative morbidity to synthetic mesh. Long-term follow-up is necessary in order to know whether the clinical outcomes of Strattice are equivalent to standard synthetic mesh in patients undergoing Lichtenstein's hernioplasty.

“…Three studies reported the surgical times and two of them contributed to the pooled analysis, except the study did by Bochicchio et al 23,25,26 They reported the mean procedure time was 134 min in the biologic group and 115 min in the synthetic group, there was no statistical different. 25 While, from the result of our study, we found that the operating time was significant longer in the biologic mesh group.…”

“…Unlike the synthetic plastic meshes, biological materials derived from human or animal sources are degraded gradually, inducing neovascularization and colonization by host cells that progressively cause a site‐specific remodelling process until reconstruction of a new and mature autologous fascia is complete . The ability to be remodelled makes the new materials theoretically attractive to surgeons as a means to reduce post inguinal herniorrhaphy complications …”

“…The long-term durability of AlloDerm for laparoscopic groin hernia repairs is yet to be determined, but based on current data it appears rational to use this technique in laparoscopic repair of complex groin hernias where infection is suspected or inadequate prosthetic coverage with peritoneum is anticipated. An early report of a randomized comparative clinical trial of Strattice™ Reconstructive Tissue Matrix (LifeCell Corp., Branchburg, NJ, USA) and lightweight synthetic mesh in the repair of open inguinal hernias shows promising results 21 . Whether the pliability of Strattice and its increased stiffness compared with AlloDerm will make it more difficult to place laparoscopically and fixate in the inguinal region remains to be answered.…”
