Ectopic Prostatic Tissue of the Anal Canal Presenting With Rectal Bleeding

Abstract: Prostatic heterotopia is significant in several respects. Either it may be an important cause of hematuria or unusually, as in our case, it may cause rectal bleeding. In addition, ectopic tissue may be endoscopically confused with malignancy in either urinary or lower gastrointestinal system. This and other reports may disclose the genesis and significance of this peculiar tissue remnant.

“…Ectopic prostate tissue has previously been reported in the lower gastrointestinal tract such as anal canal. 4 , 7 However, this case is unique because it’s the first description of the presence of ectopic prostatic tissue in mural of the rectum. This is a highly unusual finding and probably reflects abnormal embryogenesis or divergent differentiation.…”

“…In addition, it may be confused with malignancy in the gastrointestinal system, or retrovesical space. 3 , 4 Usually, surgical treatment by simple extirpation of the tumor is recommended and is curative. 9 There is possibility that ectopic prostatic tissue malignant transformation can happen 10 , however microscopic examination of the specimen in our case has not revealed any sign of malignancy.…”

“…Because lesions of mammary glands and sweat glands stain for ER and GCDFP-15, immunohistochemistry may not be used to distinguish hidradenoma papilliferum from mammary lesions [8]. Phyllodes tumor has been found in ectopic prostate tissue, mostly in female patients, the most common locations being the vulva, vagina, cervix, urinary bladder and anal canal [11-13]. Gynecomastoid hyperplasia around the main lesion may cause a pathologist to suspect residual normal prostate glands.…”

“…At rectal examination, a 2.5-cm bleeding sessile polypoid mass was found in the anal canal and surgically removed. Pathologic evaluation confirmed the prostate as a source [15]. Another rare case of extra-urinary prostatic ectopia was described in a 64-year-old man presenting with dysuria secondary to an ectopic prostatic mass posterior to the bladder [16].…”
