Effect of Administration of Activated Charcoal and Fibre on Absorption, Excretion and Steady State Blood Levels of Digoxin and Digitoxin. Evidence for Intestinal Secretion of the Glycosides

Abstract: ABSTRACT. The effects of activated charcoal and fibre on absorption and stationary plasma levels of digoxin and digitoxin have been studied. The effect of fibre alone is small but an interactive effect on absorption may occur if fibre and digoxin are ingested simultaneously. Charcoal effectively decreases glycoside absorption even when administered after the glycosides. During maintenance therapy with digoxin or digitoxin, charcoal administration decreased the glycoside plasma levels by 31.2% and 18.3% respec… Show more

“…Wheat bran is the seed coat of the grass plant wheat, which harmonizes the middle and benefits the spleen ( Wu et al, 2014 ). Wheat bran itself has little effect on digoxin, but if ingested at the same time as digoxin, it may form a bond with digoxin, which affects the absorption of digoxin and reduces digoxin plasma concentration (31.2% less than digoxin alone), thus, reducing the bioavailability of digoxin ( Reissell and Manninen, 1982 ).…”

“…Because of changes in carbohydrate and glucose absorption, dosing of antidiabetic agents may require modification and therefore patients in this population should be monitored when taking bulk laxatives [254][255][256][257][258][259][260][261]. Bulk laxatives appear to have some effect on the absorption of orally administered medications, which can result in changes in drug plasma levels [262][263][264][265][266][267][268][269][270][271][272]. For example, in one study the effect of guar gum on digoxin and phenoxymethyl penicillin absorption was studied in 10 healthy volunteers, with significant reductions in both peak penicillin plasma concentrations and AUC, but little effect on overall digoxin levels [269].…”
