Background/aim: This study aimed to investigate the effects of two different medical treatment options on choroidal thickness (CT) in patients with primary hypertension. Materials and methods: Forty newly diagnosed primary hypertension patients and 21 healthy volunteers were included. The patients were randomly divided into two subgroups. Group I started on perindopril arginine and Group II started on amlodipine. Submacular CT using optical coherence tomography (OCT) was measured before treatment and at the third and sixth months after treatment. Results: Initial mean arterial pressure (MAP) values in Groups I and II and the control group were 113.4, 109.8, and 89.4 mmHg, respectively, and mean CT values were 257.9, 286.5, and 300.9 µm. Mean MAP values in Groups I and II and the control group at the sixth month after treatment were 99.7, 99.6, and 90.2 mmHg, respectively, and mean CT values were 293.1, 286, and 297.4 µm. Analysis of the changes occurring during the study revealed significant variation in MAP in Groups I and II, and in CT in Group I only. Conclusion: A gradual increase in CT developed with perindopril arginine therapy in patients with primary hypertension, while no significant change occurred in CT in the amlodipine group.

Objetivo: Estudar os efeitos de duas drogas utilizadas no tratamento de hipertensão arterial sistêmica (captopril e propranolol) sobre a pressão intra-ocular (PIO) e pressão de perfusão (PP) em cães anestesiados. Métodos: Foram estudados 24 cães, divididos em 3 grupos de 8. No primei ro grupo (GI), foi administrado captopril (um inibidor da enzima conversora de angiotensina) na dose de 1,5 mg/kg por via endovenosa. No segundo grupo (GII), foi administrado propranolol (um beta-bloqueador) na dose de 1,5 mg/kg por via endovenosa. O terceiro grupo (GIII) foi o grupo controle. A PIO e a pressão arterial média (PAm) foram medidas por manometria. A pressão de perfusão (PP) foi calculada pela diferença entre a PAm e a PIO. A freqüência cardíaca (FC) foi monitorada com oxímetro de pulso. Os parâmetros foram estudados em 6 momentos (0, 10, 30, 60, 90 e 120 minutos). Resultados: Houve redução estatisticamente significativa da PIO (p<0,05) nos grupos em que foram administrados captopril e propranolol, sem diferença entre as drogas. Com captopril, houve redução da PAm e da PP aos 10 e 30 minutos. Com propranolol, não houve redução da PAm ou da PP. Conclusão: Houve redução da PIO com uso do captopril e também do propranolol. Entretanto, a redução acentuada da PAm e da PP causadas pelo captopril, podem ser indesejáveis para a irrigação do nervo óptico. O glaucoma é considerado uma neuropatia óptica anterior, acompanhada geralmente de pressão intra-ocular elevada, caracterizada por alterações campimétricas, escavação e atrofia da papila (1) . A pressão intra-ocular (PIO) ainda é considerada o principal fator de risco para a doença. Segundo Sommer (2) há uma relação de "dose-resposta" entre o fator de risco (PIO) e a doença, ou seja, quanto maior o nível da pressão, maior o risco de ocorrer glaucoma. Entretanto, os mecanismos que levam à atrofia do nervo óptico permanecem desconhecidos (3) . Além da PIO, vários autores consideram que fatores circulatórios estejam envolvidos nos mecanismos de lesão do nervo óptico. Trabalhos com doppler colorido demonstraram diminuição do fluxo sangüíneo e aumento da resistência vascular na artéria oftálmica em pacientes com glaucoma em relação a indivíduos normais (4)(5)(6) .
