Effect of danshen extract on the activity of CYP3A4 in healthy volunteers

Abstract: WHAT IS ALREADY KNOWN ABOUT THIS SUBJECT• Many cytochrome P450 mediated interactions have been reported between drug and herbal medicines. In particular, CYP3A4 is known to play a critical role in several clinically relevant herb-drug interactions. Midazolam has been reported to be one of the preferred in vivo CYP3A probes.• It has been reported that the lipophilic components of danshen could induce CYP3A in C57BL/6J mice, that cryptotanshinone and tanshinone IIA of danshen extract could activate human PXR and… Show more

“…The metabolic capability of CYP2B6 was 13.4 ± 5.2, which was in agreement with reported data (6.3-15.3). After an oral dose of this cocktail, the dose-normalized AUCs of MDZ (1.0 ± 0.3 h·kg/L) and its metabolite OH-MDZ (0.32 ± 0.09 h·kg/L) were similar to the documented data obtained in healthy Chinese [23] and Caucasian volunteers [25]. The AUC ratio of OH-MDZ to MDZ was 0.34 ± 0.09, which did not show a big difference from reported data (0.22-0.30).…”

“…Case reports have highlighted the possibility of interactions between warfarin and danshen, resulting in an increased anticoagulant effect [41,42,43,44]. A pharmacokinetic mechanism seems unlikely since danshen has been shown to induce intestinal CYP3A4 in 14 healthy volunteers [45]. …”

“…A randomised, open-label, controlled study of newly diagnosed ischaemic stroke patients (n = 35) found that two weeks of concomitant use of Panax ginseng extract 1.5 g with warfarin did not significantly affect peak values, INR and PT area under the curve, thus concluding that Panax ginseng did not influence the pharmacological action of warfarin.
