BackgroundSerum lipid profile changes have been observed during malaria infection. The underlying biological mechanisms remain unclear. The aim of this paper is to provide an overview on those serum lipid profile changes, and to discuss possible underlying biological mechanisms and the role of lipids in malaria pathogenesis.MethodsA systematic review and meta-analysis to determine lipid profile changes during malaria was conducted, following PRISMA guidelines. Without language restrictions, Medline/PubMed, Embase, Cochrane Central Register of Controlled Trials, Web of Science, LILACS, Biosis Previews and the African Index Medicus were searched for studies published up to 11 July, 2013, that measured serum lipid parameters in malaria patients. Also, major trial registries were searched. Mean differences in lipid profile parameters were combined in fixed and random effects meta-analysis, with a separate analysis for different groups of controls (healthy, other febrile illnesses or very low parasitaemia). These parameters were also compared between severe malaria and uncomplicated malaria. Funnel plots were used to test for publication bias.ResultsOf 2,518 studies reviewed, 42 met the criteria for inclusion in the qualitative analysis, and of these, 15 reported the necessary data for inclusion in the meta-analysis for cholesterol; nine for high-density lipoprotein (HDL), eight for low-density lipoprotein (LDL), and nine for triglycerides, respectively. Total cholesterol, HDL and LDL concentrations were lower in malaria and other febrile diseases compared to healthy controls. The decline was more pronounced and statistically significant during malaria compared to other febrile diseases. These results were consistent across included studies. Triglycerides were raised compared to healthy controls, but not statistically significant when compared to symptomatic controls.ConclusionsThis meta-analysis suggests that the observed lipid profile changes are characteristic for malaria. Although a definite link with the pathogenesis of malaria cannot yet be demonstrated, plausible hypotheses of biological mechanisms involving host lipid alterations and the pathogenesis of malaria exist. An increased research effort to elucidate the precise pathways is warranted, since this could lead to better understanding of malaria pathophysiology and consequently to novel treatment approaches.

L’infection palustre est une parasitose qui s’accompagne assez fréquemment de perturbations hématologiques et biochimiques. Quoique non spécifiques, ces signes biologiques peuvent être évocateurs du paludisme chez des patients ayant séjourné en zone d’endémie et présentant des signes cliniques de la maladie. Nous avons essayé à travers cette étude d’apprécier la fréquence de ces perturbations chez une série de cas avec paludisme d’importation. Nous avons analysé la NFS, le bilan lipidique, la CRP et la LDH chez une série de 30 cas de paludisme. La thrombopénie était présente chez 90% des cas (n=27) et l’anémie chez 23% des cas (n=7). La CRP était augmentée chez tous les cas. L’hypocholestérolémie était chez 80% des cas (n=24), l’hypoHDLémie chez 93,3% des cas (n=28), l’hypertriglycéridémie chez 36,6% (n=11) et la LDH était élevée chez 53,3% des cas (n=16). Ainsi, devant une suspicion d’infection palustre avec un examen direct négatif, la présence associée de ces perturbations biologiques augmenterait la probabilité diagnostique en faveur du paludisme.Malaria infection is a parasitic disease often associated with hematological and biological disturbances. Although not specific, these biological signs may be suggestive of malaria in patients who stayed in endemic areas and showing clinical signs of the disease. Our study aimed to assess the frequency of these disturbances in a series of cases with imported malaria. We analyzed NFS, the lipid profile, CRP and LDH in a series of 30 cases of malaria. Thrombopenia was found in 90% of cases (n = 27) and anemia in 23% of cases (n = 7). CRP increased in all cases. Hypocholesterolemia was found in 80% of cases (n = 24), hypoHDLemia in 93.3% of cases (n = 28), hypertriglyceridemia in 36.6% (n = 11) and LDH increased in 53.3% of cases (n = 16). Thus, where there is suspicion of malaria infection with negative direct examination, the presence of both these biological disturbances would increase the diagnostic probability of malaria.

Identification of natural peptides as a new class of antimalarial drugs by i in silico i approaches

Malaria is one of the most widespread and serious parasitic diseases worldwide. Currently available antimalarial drugs have side effects, and many strains of have developed resistance to such drugs. The present review examines the use of annexins and of natural peptides from snake venom as a new class of anti-malarial agents, with the key property of reducing inflammation. Severe cases of malaria manifest elevated serum levels of liver enzymes, inflammation, fibrin deposition, apoptosis, and reduction in peripheral CD8 T cells. The annexin-A1/5 proteins trigger inflammation via increased expression of diverse cytokines (tumor necrosis factor alpha, interleukin-1 beta, interleukin-10), however, by shielding microbial phospholipids they prevent injury via damage-associated molecular patterns (DAMPs). Here, we also review an -based bioengineering approach that may allow for a better design, synthesis and characterization of novel peptides from snake venom as a more effective approach to treatment due to their improved antimalarial activity.
