Background Osteoarthritis (OA) is one of the most common musculoskeletal disorders present worldwide. It increases with age and is prevalent among elderly people affecting their daily activities thus adding to the economic burden. Objectives This clinical study was undertaken to assess the efficacy of homeopathic medicines in the treatment of OA. Methods This single-blind, randomised, placebo-controlled, clinical trial was conducted on 90 patients suffering from OA. Each patient was randomised into an intervention group (IG) and a control group (CG). The IG (n = 60) received individualised homeopathic medicine (IHM) based on the symptom similarity of the case. The CG (n = 30) received an identical-looking placebo. Outcome assessment was done by assessing the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) scores at the baseline and after 3 months of follow-up. Paired t-test was used to determine differences between before and after treatment within groups. Results The difference in the WOMAC index score before and after treatment in the CG was found non-significant (p = 0.96), while in the IG the difference was statistically significant (p < 0.05) at a 95% of confidence level. The most useful medicines indicated were Bryonia alba, Medorrhinum, Pulsatilla pratensis, Rhus toxicodendron, Arnica montana, Causticum and Sulphur. Conclusion Patients with OA reported a significant decrease in WOMAC index score after 3 months of homeopathic treatment based on the totality of symptoms.

Introduction This study aimed at examining the feasibility issues of comparing individualized homeopathic medicines (IHMs) with identical-looking placebos for treating knee osteoarthritis (OA). Methods Forty eligible patients participated in this double-blind, randomized (1:1), placebo-controlled feasibility trial in the outpatient clinics of a homeopathic hospital in West Bengal, India. Either IHMs or identical-looking placebos were administered, along with mutually agreed-upon concomitant care guidelines. The Knee Injury and Osteoarthritis Outcome Score (KOOS) was the primary outcome measure, and derived Western Ontario and McMaster Universities Arthritis Index (WOMAC) scores from KOOS, EQ-5D-5L questionnaire, and Visual Analog Scale (VAS) were the secondary outcomes; all measured at baseline and after 2 months. Group differences and effect sizes (Cohen's d) were estimated using an intention-to-treat approach. p-Values less than 0.05 (two-tailed) were considered statistically significant. Results Enrolment/screening and trial retention rates were 43% and 85% respectively. Recruitment was difficult owing to the coronavirus disease 2019 (COVID-19) lockdown. Group differences were statistically significant, favoring IHMs against placebos in all the KOOS sub-scales: symptoms (p < 0.001), pain (p = 0.002), activities of daily living (p < 0.001), sports or recreation (p = 0.016), and quality of life (p = 0.002). Derived WOMAC scores from KOOS favored IHMs against placebos: stiffness (p < 0.001) and pain (p < 0.001). The EQ-5D-5L questionnaire score (p < 0.001) and EQ-5D-5L VAS scores (p < 0.001) also yielded significant results, favoring IHMs over placebos. All the effect sizes ranged from moderate to large. Sulphur was the most frequently prescribed homeopathic medication. Neither group reported any harm or serious adverse events. Conclusion Although recruitment was sub-optimal due to prevailing COVID-19 conditions during the trial, the action of IHMs was found to be superior to that of placebos in the treatment of knee OA. Larger and more definitive studies, with independent replications, are required to substantiate the findings. Trial registration: CTRI/2021/02/031453.

Objectives: To investigate the role of individualized homoeopathic medicines in treatment of the Musclo Skeletal Disorders (MSS) of the elderly. Materials and Methods: An Observational cohort study was done by the secondary data analysis of 278 patients (age group of 60-65 years) visiting the Homoeopathic hospital from a period of 10 years (2000)(2001)(2002)(2003)(2004)(2005)(2006)(2007)(2008)(2009)(2010) to observe the subjective relief with homoeopathic medicines in the Musclo Skeletal System Disorders (MSS). Result: In the study after using homoeopathic medicines, 7.32% of patients reported 25% relief, 35% of patients had 50% relief, 17% patients were better by 75%, 14% patients were free of pain and ease in mobility, while 16% patients reported no relief. Homoeopathic medicines like Kali Carb (27.3%), Silicea (21.2%), and Calcarea Flour (13.7%) were the most indicated remedies in the elderly patients with MSS complaints and centesimal scale (30C -38% of patients & 200C -59% of the patients) was the most common potency scale used. Conclusion: Homoeopathy is effective in offering subjective sense of relief in elderly patients from their MSS complaints.
