This study used real-world data (RWD) to explore the long-term effects of East Asian traditional medicine (EATM) on heart failure (HF). A comprehensive search was conducted across five databases to identify relevant studies, which were then reviewed using the Arksey and O’Malley scoping review framework. The analysis focused on a descriptive examination of the long-term outcomes associated with EATM intervention. Methodologically, the study explored various aspects, including study subjects, interventions, applied clinical outcomes, and statistical methods. Out of 258 studies, 12 were selected. Eight studies involved patients with HF, while the others used HF as an outcome. Datasets from the National Health Insurance Research Database were used in Taiwan, while electronic medical record data were used in China and Japan. EATM interventions have been found to be associated with lower mortality and readmission rates. One study indicated that an increased dose of Fuzi, a botanical drug, or prompt use of Fuzi after diagnosis led to a decreased mortality hazard ratio. In two studies examining readmission rates, a significant increase was observed in the non-exposed group, with odds ratios of 1.28 and 1.18. Additionally, in patients with breast cancer, the subdistribution hazard ratio for the occurrence of doxorubicin-induced HF was reduced to 0.69. Although cohort studies with survival analysis were common, methodological flaws, such as issues with statistical methods and HF diagnosis, were identified. Despite these challenges, the study observed an association between EATM and improved clinical outcomes in patients with HF, emphasizing the potential of RWD studies to complement randomized controlled trials, especially for longer-term follow-ups. These results provide foundational data for future RWD research.

Purpose: To apply bioinformatics for the identification of potential genes associated with Huo Xin Pill (HXP), a traditional Chinese medicine (TCM) used for the treatment of acute myocardial infarction AMI).Methods: Mouse AMI expression profile dataset GSE153485 and HXP-treated mouse AMI expression profile dataset GSE147365 were downloaded from GEO database. Then, R software was used to screen differentially-expressed genes in AMI and differentially-expressed genes in HXP-treated AMI. Gene Ontology (GO) enrichment analysis, Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analyses, Venn diagrams, and protein-protein interaction (PPI) analysis were carried out on the hub genes linked to the effect of HXP on AMI.Results: Six hub genes were identified. Based on the differential analysis of the sham and AMI groups, GSE153485 and GSE147365 had 840 and 2116 differentially-expressed genes, respectively (p < 0.05). The GO and KEGG analyses revealed enrichments in actin filament organization, membrane repolarization, and regulation of the actin cytoskeleton. Differential analysis of the use of HXP on AMI showed that GSE147365 had 380 differentially-expressed genes, comprising 96 up-regulated genes and 284 down-regulated genes (p < 0.05). Thirteen potential acting target genes were obtained using a enn diagram, while 6 key acting genes were obtained via final screening.Conclusion: Six (6) hub genes linked to HXP and AMI have been identified using bioinformatics: Egr2, Tubb2a, Col4a2, Cnn2, Lmna, and Col4a1. This study provides a partial experimental basis for the use of HXP in the treatment of AMI. In addition, it provides new potential targets for the treatment of AMI.

Heart failure (HF) is a major global healthcare problem leading to substantial deterioration of prognosis. The current clinical guidelines in HF have begun to emphasize the importance of traditional Chinese medicine (TCM). TCM has been utilized in clinical practice for over 2000 years and is capable of treating a variety of HF pathogenic issues. This review summarizes the classification, pathophysiology, and treatment of HF from three perspectives: Western medicine, TCM, and integrated traditional Chinese and Western medicine (ITCWM). Emphasizing the most recent evidence, this review consolidates knowledge on ITCWM treatments for HF pertaining to different TCM syndromes, including TCM decoctions, oral patent Chinese medicine, TCM injections, as well as therapies like acupuncture and moxibustion. Additionally, this review explores TCM approaches to HF prevention, such as tai chi, Baduanjin exercise, and Sanfu acupoint herbal patching. The findings of this study suggest that ITCWM holds promise for the treatment and rehabilitation of HF. However, further research is warranted to elucidate the underlying mechanisms.
