Effect of Shunt Operation on Idiopathic Normal Pressure Hydrocephalus Patients in Reducing Caregiver Burden: Evidence from SINPHONI

Abstract: Background/Aims: Patients with idiopathic normal pressure hydrocephalus (iNPH) are often given shunt operations to reduce the triad symptoms (cognitive impairment, gait disturbance and urinary disturbance). We examined whether they also reduce caregiver burden. Methods: The personal strain (PS) and role strain (RS) factors, which are related to the stress and constraints, respectively, on the caregivers of 81 iNPH patients were evaluated with the Zarit burden interview (ZBI) and each of the triad symptoms was … Show more

“…Our results differ from those of Kazui et al, who found a decreased total burden after treatment according to the Zarit Burden Interview (ZBI). 16 The difference could be due to differences in sensitivity between the 2 scales, but there is no validated Swedish version of ZBI.…”

“…There is doubt if depressive symptoms are consequence to a primary disturbance in frontal neurotransmitter machinery 12 or if symptoms are a result of self conscience of limited motor, continence and cognitive abilities, representing a burden to himself and to the family, thus leading to chronic sad humor 18 . Unusual appearances of NPH symptoms may hinder early diagnosis and consequently proper treatment 5,6,7 .…”

“…The clinical manifestations are gait disturbance, dementia, and urinary incontinence without preceding disorders. 1) Patients with these symptoms often need care to cope with the activities of daily living, 8,21) so adequate diagnosis and treatment (e.g., ventriculoperitoneal shunt or lumbo-peritoneal shunt) 28) are essential to reduce medical and social-welfare costs in countries with aging populations. The diagnosis is based on clinical observations supported by computed tomography (CT) and magnetic resonance (MR) imaging, and other neuroradiographical examinations.…”
