Neurological complications related to multiple myeloma (MM) are not uncommon; however, direct involvement of the central nervous system (CNS) is extremely rare and represents a diagnostic and therapeutic challenge. Significant survival difference has been noted with the introduction of novel therapy in patients with MM, but their effect on the incidence and their use for management of leptomeningeal myeloma (LMM) is uncertain. Analysis of published data demonstrates its recent increased incidence, median time to CNS presentation, and slight improvement in median survival after diagnosis of LMM. Less common MM isotypes have been overrepresented in LMM. CNS relapse occurred mostly in patients with Durie-Salmon stage III MM. Despite treatments, standard or experimental, the survival rates of LMM remain dismal. Monitoring high risk patients closely, even after achieving complete remission, may be useful in early detection of LMM. As we gain better understanding of LMM, we recommend that future research and clinical care focus on earlier diagnosis and development of more efficient CNS-directed therapy to improve survival in this patient population.

Multiple myeloma (MM), one of the B-cell non-Hodgkin lymphomas, is a bone marrow-derived, antibody-producing cancer of the plasma cells. In the advanced stages, the cancer cells frequently cause widespread osteolytic bone damage; however, in rare cases, MM also manifests as an intracranial plasmacytoma. In the present study, we describe a case in which a patient, initially treated for MM and with subsequent complete remission, was admitted to hospital with a lesion in the right cerebellar hemisphere and neurological symptoms of a brain tumor. Our initial diagnosis was an MM relapse with the rare occurring intracranial manifestation. However, pathological tests confirmed the diagnosis of a high-grade astrocytoma. In this case report, we describe the characteristics, as well as the treatment issues, diagnoses and clinical developments of this patient.

Central nervous system (CNS) involvement is extremely rare and comprises 1% of cases. Review of literature shows only limited data on this subject and consists mostly of few case reports. Scarce knowledge about CNS involvement in the course of multiple myeloma makes its diagnosis and treatment very difficult and challenging. The authors describe a clinical case which illustrates the problem from a clinician point of view. Furthermore, we analyzed the available data from literature and summarized the current state of knowledge on diagnosis, potential prognostic factors, treatment and prognosis in CNS myeloma involvement.
