Effects of Digoxin in Heart Failure (HF) With Reduced Ejection Fraction (EF)

Abstract: In this review, we evaluated the literature on the benefits and deleterious effects of digoxin in heart failure (HF) with reduced ejection fraction (EF). Although digoxin was considered an effective treatment for HF, the supporting evidence is conflicting. Before the conventional use of modern HF therapies, digoxin was widely used for symptomatic relief on these patients. Further randomized trials are required to reach a definite conclusion about its efficacy and safety in patients experiencing HF with a reduc… Show more

“…Although digoxin is no longer considered a first line treatment for congestive heart failure ( 39 ), it is still being used for rate control in atrial fibrillation when hypotension develops due to attempts to increase the dose of other cardiac medications ( 40 ). With the aging population, atrial fibrillation is becoming a “global epidemic” ( 41 ).…”

“…This is especially true for cardiac glycosides, which can be applied in clinical practice only cautiously due to their very narrow therapeutic window. The use of digoxin is well-explained in heart failure (HF) patients with reduced ejection fraction; however, the supporting evidence is conflicting [ 4 ]. A recent systematic review and meta-analysis reported that digoxin has no effect on the all-cause mortality and hospitalization in heart failure with preserved ejection fraction patients [ 5 ].…”
