Effects of a multimodal approach to food art therapy on people with mild cognitive impairment and mild dementia

Background Food art therapy (FAT) has multiple modalities in which cognition, emotion, and social changes are stimulated. The purpose of this study was to design a multimodal approach to a food art therapy (MM‐FAT) program and identify its effects on cognitive ability, daily living functioning, depression, self‐esteem, self‐efficacy, self‐expression, and social functioning in individuals with mild cognitive impairment (MCI) and mild dementia by employing a mixed methods research design. Methods The participants included 39 patients from a public dementia care centre in Seoul, Korea. The intervention group, which comprised 20 participants, received 12 MM‐FAT sessions 3 times a week for 4 weeks, and the control group, which included 19 participants, received usual care. The MM‐FAT program was evaluated based on its effectiveness on cognitive, daily living, emotional, and social functioning outcome measures at three time points using repeated measures analysis of variance. Semi‐structured interviews (n = 9) were conducted to evaluate the overall experience of the MM‐FAT program and its outcomes. Results The findings reveal that MM‐FAT has a positive effect on the cognitive, emotional, and social functioning of individuals with MCI and mild dementia. However, there were no enhancements in individuals' daily living functioning, and the lasting effects of the intervention could not be assessed. Cognition and depression increased significantly at the end of the MM‐FAT program. Self‐expression and self‐efficacy were significantly higher in the MM‐FAT group than in the control group. The semi‐structured interviews revealed improvements in participants' behaviour, communication, and interaction. Conclusion This mixed methods study focused on individuals with MCI or mild dementia contributes to an understanding of the effectiveness of a FAT program employing a multimodal approach. Despite the COVID‐19 pandemic, the study was able to enrich the effects of MM‐FAT on cognitive, emotional, and social functioning through qualitative findings.

Background: Mild cognitive impairment (MCI) is a stage of cognitive ability loss with intact activities of daily living and an increased risk for the development of dementia. Objective: This study evaluated the intervention effect of remote expressive arts program (rEAP) on cognitive function in older adults with MCI and investigated the underlying neurobiological mechanisms. Methods: We assigned 73 older MCI patients to receive rEAP or health education (HE), who underwent neuropsychological evaluation and resting-state functional magnetic resonance imaging before and after treatment. Neuropsychological scores were analyzed using SPSS software, and regional homogeneity (ReHo) values and seed-based functional connectivity (FC) were analyzed using Matlab software. Results: The rEAP group showed more significant improvements in cognitive function than the HE group. rEAP affected spontaneous brain activity and brain networks. The ReHo values in the right anterior cingulate/paracingulate cortex and the left dorsolateral superior frontal gyrus significantly increased and decreased, respectively, in the rEAP and HE groups. Further, ReHo value changes were significantly associated with the corresponding neuropsychological test score changes in the rEAP group. Moreover, the rEAP group showed decreased FC between the posterior cingulate cortex and the right middle temporal gyrus and increased FC between the ventromedial prefrontal cortex and left angular gyrus. Conclusion: The 12-week rEAP improved cognitive function in MCI patients. Additionally, the alterations of spontaneous brain network connections and activity helped improve and maintain cognitive function in MCI patients.

IntroductionMild cognitive impairment (MCI) is a known risk factor for the development of dementia. The potential benefits on cognition from non‐pharmacological measures such as art‐based interventions are of increasing interest. This systematic review examines the evidence for the impact of one form of art‐based intervention, visual art therapy (VAT), on the cognition and psychological wellbeing of older people with MCI.MethodsRandomised controlled and quasi‐experimental trials evaluating the efficacy of VAT in older persons aged over 60 years with MCI were included. A search was performed on electronic databases: MEDLINE, CINAHL, Embase and PsycINFO. Joanna Briggs Institute critical appraisal and extraction tools were utilised for risk of bias assessment and data extraction, respectively. A narrative descriptive approach was used to outline the findings.ResultsSeven studies were identified from 4311 articles screened. Improvement in cognition was reported in five studies, with two of these reporting sustained improvement at 6–9 months, while the remaining three studies showed improvement only at the immediate post‐intervention period. A positive impact was reported in four of six studies that examined the effect of VAT on participant psychological wellbeing. The overall methodological quality of the studies ranged from moderate in four of five RCTs, to high in the quasi‐experimental studies and one RCT. However, the low study power in the context of small sample sizes limits the applicability of these studies to the population of interest.ConclusionsVAT is potentially an effective non‐pharmacological intervention that may enhance cognition and provide benefits for psychological wellbeing in older persons with MCI. Given the limited studies available, with the majority emerging over the last 5 years, further research is required to confirm these reported benefits, as well as to determine whether VAT impacts on the progression of cognitive decline in MCI.
