Effects of Mirtazapine for Methamphetamine Use Disorder Among Cisgender Men and Transgender Women Who Have Sex With Men

Abstract: IMPORTANCE Methamphetamine use is increasingly prevalent and associated with HIV transmission. A previous phase 2a study of mirtazapine demonstrated reductions in methamphetamine use and sexual risk behaviors among men who have sex with men.OBJECTIVE To determine the efficacy of mirtazapine for treatment of methamphetamine use disorder and reduction in HIV risk behaviors. DESIGN, SETTING, AND PARTICIPANTSThis double-blind randomized clinical trial of mirtazapine vs placebo took place from August 2013 to Septem… Show more

“…Medications to treat opioid use disorder are highly effective 25,26 . The efficacy of treatments for stimulant use disorder—including cognitive behavioral therapy, contingency management, and pharmacological treatment with mirtazapine—have been more modest 27‐30 . Even in the context of high levels of health insurance coverage (ie, Washington is a Medicaid‐expansion state), only a minority of PWID whose main drug was goofball reported any current treatment.…”

“…In men who have sex with men, the antidepressant mirtazapine reduced MA use and high-risk sexual behaviours, despite low medication adherence rates [30]. In another study published by this group since our search, 120 cisgender males and transgender females who had sex with men and had MA use disorder were randomly assigned to mirtazapine 30 mg or placebo OD for 24 weeks with a further 12 weeks' follow-up [71]. The primary outcome was MApositive urines and secondary outcomes were sexual risk behaviours.…”

“…Injection drug use is a significant driver of HCV and HIV transmission through high-risk practices of sharing needles, syringes, and other drug injection equipment, as well as from associated risk environments [2,3]. A focus on PWID and efforts to address substance use, Fig 2. (a) Prevalence of current injection drug use by age and race among 2,200 surveyed emergency department patients.…”

“…The high-risk practice of sharing needles, syringes, and other drug injection equipment put PWID at high risk for acquiring and transmitting human immunodeficiency virus (HIV) and hepatitis C virus (HCV) infection. Additionally, PWID are exposed to significant adverse risk environments that can lead to HIV and HCV infections, including homelessness, high-risk sexual practices, and incarceration [2,3]. PWID account for 12% of new HIV and 60% of new HCV infections in the United States (US) annually, and there has been a marked increase in HCV transmission directly related to the opioid epidemic and PWID [4,5].…”
