Effects of the COVID-19 Pandemic on Anti-vascular Endothelial Growth Factor Treatment in China

Abstract: We evaluated the impact of the COVID-19 pandemic on anti-VEGF treatment in ophthalmology patients in a single hospital in northern China. A total of 93 anti-VEGF injections were administered to 85 eyes of 72 patients at The China Medical University First Hospital Department of Ophthalmology during the COVID-19 pandemic. Compared to the same period in 2019, the number of injections decreased by 70%. Fifty-nine eyes of 46 patients were receiving 3+PRN anti-VEGF treatment prior to the outbreak of the COVID-19 pan… Show more

“…During the lockdown, the national healthcare system focused on the prevention and management of COVID-19-related disease and emergency services only. In the majority of ophthalmology clinics in Greece, regular clinic visits, elective surgeries, scheduled intravitreal injections, and non-urgent eye conditions were deferred, as it also occurred in most countries worldwide [4][5][6][7][8]. In addition, many patients have themselves postponed their visits to ophthalmology clinics in order to avoid being exposed.…”

“… 9 The results of the study evaluating the impact of the COVID-19 pandemic on anti-VEGF treatment in ophthalmology patients also demonstrated that longer treatment interruption was associated with worsened visual acuity. 18 Again, these findings emphasize the importance of normative, adequate course of anti-VEGF therapy in patients with CRVO-ME.…”

“…Anatomic and functional outcomes suggest that patients with anti-VEGF treatment interruptions are at risk for severe adverse visual sequelae. 18 In the CRUISE trial, 15.4% of patients in the group that received sham injections lost ≥15 letters (3 lines) from their baseline BCVA letter score at month 6, which is considered similar to the natural loss of visual acuity over a 6-month period. 5 A recent meta-analysis conducted for 24 studies determined that the pooled mean decrease was 10 letters in visual acuity from baseline to 6 months and 3 letters from baseline to ≥12 months for eyes with non-ischemic CRVO.…”

“… 37 In addition, a Chinese study showed that interruption of anti-VEGF treatments exposes patients at significative risk for severe adverse visual sequelae. 38 A USA team recently demonstrated that patients who missed a visit for intravitreal injection lost more vision than patients who completed the scheduled visits (no visit −5.024 ±1.88, canceled visit: −1.633 ±0.65, completed visits 0.373 ±0.50; p = 0.0028) so that a delay in care of even only 5.34 weeks resulted in vision loss (Italian lockdown lasted 9.857 weeks). 39 The population of diabetic subjects treated in Italy with intravitreal injection was estimated, starting from the fact that about 30% of the total intravitreal injections are administered to diabetic patients by an average of eight injections per year.…”
