Background Gestational Diabetes Mellitus (GDM) is a prevalent obstetric complication that impacts both maternal and neonatal health by increasing the risk of adverse outcomes such as preterm birth and macrosomia. Traditional health education methods for GDM lack in clinical efficacy due to the absence of timely evaluation and personalized feedback, a gap attributed to the insufficient integration of nursing theories and quality management tools. This study aims to explore a novel approach for clinical health education in GDM patients by evaluating the efficacy of combining King’s Theory of Goal Attainment and the Failure Modes and Effects Analysis with the Plan-Do-Check-Act (FMEA-PDCA) quality management tool. Methods The study was conducted among pregnant women attending tertiary hospitals in Fujian Province from March 1, 2022, to May 31, 2023. Eligible participants were randomly divided into two groups (59 per group), via a computer-generated randomization method, to receive either an innovative health education integrating King’s Theory and FMEA-PDCA or conventional education, respectively. We measured and evaluated the changes in blood glucose, glycated hemoglobin (HbA1c), anxiety levels, quality of life, and pregnancy outcomes pre- and post-intervention. Results Following the intervention, the experimental group showed significantly lower fasting blood glucose, improved anxiety levels and quality of life (P<0.001), and a reduced rate of cesarean sections compared to the control group (P = 0.037). No significant differences were found in HbA1c levels (P = 0.671) and several pregnancy-related complications across both groups (P>0.05). Conclusion The integration of King’s Theory with the FMEA-PDCA tool in health education significantly enhances the educational quality and clinical outcomes for GDM patients, suggesting a promising strategy for clinical practice. Objective. The objective of this study is to systematically evaluate the effect of exercise on gestational diabetes (GDM). Methods. The databases of PubMed, Cochrane Library, Web of Science, Embase, CNKI, VIP, and Wanfang were searched to collect publications regarding physical exercises and GDM. The two researchers screened the literature, extracted the data, and analyzed the risk of bias of the included data using RevMan 5.3 software. The primary outcomes analyzed included the fasting blood glucose, 2-h postprandial blood glucose, glycosylated hemoglobin, premature delivery, cesarean section, neonatal macrosomia, premature rupture of membranes, and neonatal hypoglycemia. Results. A total of 9 studies with 1289 GDM patients were included. Compared with the control group, exercise could significantly reduce the 2-h postprandial blood glucose ( MD = − 0.62 , 95% CI (-0.91 to -0.34), Z = 4.29 , P < 0.0001 ), improve HbA1c( RR = − 0.47 , 95% CI (-0.81 to -0.13), Z = 2.69 , P = 0.007 ), reduce the cesarean section rate ( RR = 0.83 , 95% CI (0.71-0.98), Z = 2.25 , P = 0.02 ), and decrease the incidence of neonatal macrosomia in GDM patients ( RR = 0.57 , 95% CI (0.34-0.95), Z = 2.17 , P = 0.03 ). Conclusion. Exercise intervention can improve the blood glucose level of GDM patients, such as 2-h postprandial blood glucose and HbA1c. Meanwhile, exercise can also reduce adverse pregnancy outcomes, such as premature birth and macrosomia. Therefore, prescribing exercise to GDM patients can effectively manage GDM and improve adverse pregnancy outcomes.

A terhességi cukorbetegség az egyik leggyakoribb terhességi szövődmény, napjainkban is intenzív kutatás tárgya. Az állapot kezelése és utánkövetése az elmúlt évtizedekben nagy hangsúlyt kapott. Mindezek ellenére viszonylag kevés információ áll rendelkezésre a terhességi cukorbetegség és az ennek talaján később kialakuló 2-es típusú cukorbetegség megelőzésének lehetőségeiről. A jelen összefoglaló friss szakirodalmi adatokra támaszkodva próbál rávilágítani arra, hogy a kedvezőtlen életmódbeli tényezők nagyobb szerepet játszanak a betegség kialakulásában, mint a genetikai hajlam. Az ideális testsúly elérése és fenntartása, a szervezet inzulinérzékenységének alacsony glykaemiás indexű diétával és aerob mozgással történő javítása az egészség megőrzésének a kulcsa terhességi cukorbetegségen átesett nők esetében.
