Efficacy and Effectiveness of Lithium in the Long-Term Treatment of Bipolar Disorders: An Update 2018

Abstract: For more than 40 years, lithium has been the gold standard in the long-term treatment of bipolar disorders. In the course of the last 15 years, other drugs have been approved in this indication and are widely used in clinical practice at the expense of lithium. New research from the last few years, however, indicates that lithium is still the first-line treatment in this indication. Against this background and lithium's proven acute antimanic efficacy, we should perhaps be using lithium more regularly (in comb… Show more

“…32 Another recently updated meta-analysis of randomised controlled studies explored relapse prevention in patients with bipolar disorder, comparing lithium against placebo and anticonvulsants. 33,34 Lithium was more effective than placebo for the prevention of overall mood and manic episodes. Lithium was also more effective than anticonvulsants for the prevention of manic, but not of overall mood or depressive episodes.…”

“…Lithium is the gold standard in treating bipolar disorders, showing established effectiveness in the treatment of acute mania as well as prevention of manic and depressive episodes along with antisuicidal effects [1][2][3][4][5]. While overall prescription rates have recently decreased, possibly related to issues of toxicity and side effects, polypharmacy involving lithium has increased [1,6].…”

“…There is indeed strong evidence that these channels show distinct distribution across the different regions of the human brain (14)(15)(16)(17). The heterogeneous distribution of Li (18), and variability in Li response and tolerability is poorly understood. It is at this point that nuclear magnetic resonance (NMR) provides a unique solution to detect and quantify in a noninvasive manner the local concentration of Li ([Li]).…”
