Objective:To compare three treatment modalities in scabies for safety, efficacy, and economy in a local population of Nagpur.Materials and Methods:This was a prospective, randomized, comparative clinical trial conducted in 103 participants, randomly allocated to three groups. First group received benzyl benzoate (BB) 25% lotion, second group received permethrin 5% cream, whereas third group received tablet ivermectin 200 μg/kg as a single dose. The participants were recalled after one week for follow-up evaluation. If there were no signs of cure, the same intervention was repeated. The participants were followed up for two weeks for cure rate, adverse drug reaction (ADR) monitoring, and postintervention observation. The follow-up was stopped after two weeks.Statistics:Fischer's exact test using Graph pad Instat v 3.05.Results:Ivermectin showed 100% cure rate after two weeks of treatment. Permethrin decreased pruritus by 76% at the end of one week and had significantly better cure rate than ivermectin. At the end of two weeks treatment, this finding was reversed, that is, cure rate in ivermectin group was 100%. For cost-effectiveness analysis, treatment regimens were formulated hypothetically for comparison from Markov population tree for decision analysis. It was found that BB and ivermectin each consecutively for two weeks were most cost effective regimens giving complete cure in four weeks, while ivermectin was the fastest regimen giving the same results in two weeks.Conclusion:Benzyl benzoate as first line intervention and ivermectin in the remaining gave best cost-effective results in the study patients of scabies.

Background: The burden of scabies is highest in tropical countries, but recent data from Iraq are scanty. Objective: To estimate the frequency of scabies in patients of a dermatology clinic in central Iraq. Methodology: The study was conducted in Tikrit (Iraq), between May 2007 and February 2008. A total of 1,194 patients presenting to a dermatology clinic were included in the study. Results: During the study period, 132 [1.1%] patients with scabies were identified. The frequency was 13.5% in males and 8.6% in females [P = 0.007]. Frequency in children was 15.6% and 9.8% in adults [P = 0.007]. About 91% of cases were younger than 45 years. The treatment of scabies cases with 5% permethrin cream resulted in a cure rate of 80.3% following a single application, and was increased to 95.5% after a second application. Cure was achieved in all cases after a third application. Conclusion: Prevalence of scabies was high in a dermatology clinic, and prisoners were identified as high-risk group. Mass treatment of scabies either by oral ivermectin or topical permethrin is suggested.

:Scabies is one of the commonest diseases among all age groups. Topical permethrin is a widely used treatment option for scabies. Ivermectin is a newer oral agent for the treatment of scabies. This study was done to compare the efficacy of permithrin and oral Ivermectin in the treatment of Scabies in patient suffering from Diabetes mellitus. This comparative clinical trial was carried out in the outpatient department of Dermatology & Venereology, Diabetic Association Medical College Hospital (DAMCH), Faridpur from January 2012 to December 2012. A total 60 cases were enrolled purposively and divided into 2 groups. Group I received 2 doses oral Ivermectin and group II treated with 5% permethirn cream. Patients were followed up at the 3rd and 4th week. Total 86.6% patients of Ivermectin group and 90% of permethrin group were cured. Though permethrin showed somewhat more effective, the difference was not statistically significant. The study found that both ivermectin and permethrin were similarly effective in the treatment of scabies in patients suffering from Diabetes Mellitus.
